currently residing at 197 Trinidad St. Villa Catalina Subdivision,
Barangay San Agustin III, Dasmarinas, Cavite, after having been duly
sworn to in accordance with law do hereby depose and say:

1. That I am the Petitioner in the foregoing case.

2. That I have caused the preparation and filing of this Petition.

and attest the allegations therein are true and correct, based on
personal knowledge or based on the authentic records in my
