This slide introduces our Occupational Safety and Health (OSH) Program at Jack Fiber
Services.
Today, we'll be discussing how they ensure a safe and healthy working environment for all
their employees.

Jack Fiber Services was established in June 2016, located at 1098 Rosal St. Bambang Los
Banos Laguna, and is managed by Nuelmar Ugot.
They have a workforce of 22 employees, offering services like internet access, virtual
networks, messaging, firewall, and network security.

Their company is committed to complying with safety regulations, specifically RA 11058 and
DOLE Department Order 198-18.
They ensure this by providing necessary funding, conducting employee training, distributing
PPE, and maintaining safe working conditions.

2.0 General Safety and Health Programs
This section covers the overall approach to maintaining safety and health at our workplace.
2.1 Conduct of Risk Assessment
Risk Assessment: We regularly evaluate the potential dangers involved in different tasks, like
installing cables or surveying houses.
Identifying Hazards: For example, we look for risks such as falls, electric shocks, fiberglass cuts,
and road hazards.
Control Measures: To reduce these risks, we provide proper training, personal protective
equipment (PPE), create detailed work maps, and encourage employees to stay focused and
avoid distractions.
2.2 Medical Surveillance
Health Programs: We have several health programs to ensure employees are healthy and safe
from various health issues.
Drug-Free Workplace: We conduct tests and programs to keep our workplace free from drugs.
HIV/AIDS, Tuberculosis, Hepatitis B: We provide information, testing, and support for these
conditions to ensure everyone stays healthy.
Mental Health: We focus on mental well-being through programs and support services.
3.0 Health Programs for Promotion, Prevention, and Control
Promotion of Health: We actively encourage healthy living habits among our employees
through various programs and activities.
Preventive Measures: We take steps to prevent health issues before they arise, such as regular
health check-ups and vaccinations.
Control Measures: For existing health risks, we have measures in place to control and manage
them effectively, ensuring they don’t become bigger problems.
4.0 Additional Health Programs
Promotion Activities: These include workshops, seminars, and health campaigns to educate
employees about maintaining good health.
Prevention and Control Programs: We implement specific programs targeting common
workplace health issues, such as ergonomic assessments to prevent strain injuries and regular
screenings for early detection of diseases.

Their Safety and Health Committee includes the chairperson, Nuelmar Ugot, the secretary,
Oscar Bagtasos, and other members like Randy Garcia.
This committee develops and implements OSH policies, monitors safety programs, and
investigates any safety and health issues that arise.

They have designated safety officers like Harry Pagalangan who have completed training,
and they also have first aiders available in various departments.
Treatment rooms are provided in specific areas to handle emergencies efficiently.

It’s crucial to report major accidents to DOLE within 24 hours and submit detailed work
accident reports for other incidents within 30 days.
Keeping records of all near misses and accidents helps us improve our safety measures and
prevent future incidents.

They provide essential PPE such as hard hats, safety gloves, shoes, harnesses, and vests to
their employees to protect them from workplace hazards.
A total of 15 of each PPE item have been distributed to ensure everyone is adequately
equipped.

vehicle speeds, and employ dust suppression systems.

The annual budget for their OSH program is Php 175,000, which covers expenses for PPE,
training, signages, machine guards, medical exams, and supplies.

These assessments help us identify potential hazards like falls from heights, electric
shocks, and cuts from fiberglass. Based on these assessments, we implement control
measures including the use of personal protective equipment (PPE), proper training,
detailed work maps, and encouraging employees to stay focused to avoid
distractions.
2.2 Medical Surveillance
Our medical surveillance program is designed to monitor and support the health of
our employees. This includes initiatives to maintain a drug-free workplace, regular
health checks, and programs targeting specific health concerns such as HIV/AIDS,
tuberculosis, hepatitis B, and mental health.
By regularly monitoring the health status of our employees, we can provide early
interventions and support, ensuring that health issues do not impact workplace
safety or employee productivity.

Referencing Item 9.0 - Accident Reporting
Accident Reporting: In the event of a major accident resulting in death or
permanent total disability, we are required to report it to the Department of Labor
and Employment (DOLE) within 24 hours using the prescribed form. For other types
of work accidents that result in disabling injuries, such as permanent partial disability
or temporary total disability, we submit detailed reports to DOLE within 30 days.
Record Keeping: We maintain thorough records of all accidents, incidents, and near
misses. This documentation is crucial for analyzing trends, identifying recurring
hazards, and continuously improving our safety protocols. Our OSH Committee
regularly reviews these reports to ensure that our safety measures are effective and
up-to-date.

6.1 Safety Officer
Our designated safety officer, Harry Pagalangan, has completed 8 hours of
specialized training to oversee the OSH programs. The safety officer is responsible
for ensuring that safety policies are implemented correctly, conducting safety audits,
and providing training and guidance to employees on safety matters.
6.2 Emergency Occupational Health Personnel and Facilities
We have emergency occupational health personnel such as first aiders available in
various departments. For example, the cashier area and administration have first
aiders and treatment rooms available.
On-site installation and maintenance teams also have designated first aiders,
although treatment rooms are not available at these locations. These provisions
ensure that any medical emergencies can be promptly addressed, minimizing the risk
of severe outcomes from workplace accidents.
Slide 5: Accident/Incident/Injury Investigation, Recording, and Reporting
Accident Reporting
In the case of a dangerous occurrence or major accident, it is mandatory to report to
the DOLE Regional Office within 24 hours using the prescribed Work
Accident/Incident Notification form. This prompt reporting ensures that the
appropriate authorities are immediately aware of serious incidents and can take
necessary actions.
After investigating the incident, we prepare and submit a detailed work accident
report. This thorough documentation helps us understand the causes of accidents
and develop strategies to prevent future occurrences.
Incident Documentation
All incidents, including near misses, are recorded and reported. A systematic
approach to notification and reporting within the company allows us to track all
incidents, analyze patterns, and improve our safety measures.
The OSH Committee reviews these reports regularly to ensure that we are
continuously learning from past incidents and enhancing our safety protocols to
prevent recurrence.
Slide 6: Provision and Use of PPE
PPE Distribution
We provide a range of personal protective equipment (PPE) to our employees,
including hard hats, safety gloves, safety shoes, harnesses, and reflectorized vests.
Each of these items is essential for protecting employees from specific hazards they
might encounter in their daily tasks.
For instance, hard hats protect against head injuries, gloves and shoes protect
against cuts and punctures, harnesses prevent falls, and vests ensure visibility in
high-traffic areas.
Training
Alongside the distribution of PPE, we provide comprehensive training on the correct
use, handling, cleaning, and maintenance of this equipment. Proper training ensures
that employees not only use their PPE correctly but also maintain it in good
condition, maximizing its effectiveness in protecting them from hazards.
Regular refresher courses and hands-on demonstrations help reinforce the
importance of PPE and ensure that all employees are comfortable and confident in
using their protective gear.

We use safety signages throughout the workplace to warn workers and the public
about potential hazards. These signs are crucial for preventing accidents by alerting
everyone to dangers such as slippery floors, high voltage areas, and construction
zones.
Visibility and Placement
Safety signs are strategically placed in high-traffic and high-risk areas to ensure they
are easily seen and understood by everyone. Clear, well-placed signage helps in
maintaining a safe environment by reminding employees and visitors of the
necessary precautions they need to take.

So, sinasabi dito na workplace health and safety ay kino-considered ng management to be an
integral part of the good and safe performance ng potentially hazardous job. That is why the safety
and health committees of Jack Fiber Services, tulad ng mga nakalista dito, ay kailangan mag
perform and implement ng OSH policies and programs as required by OSH law.

So, together with the safety and health committee, safety officer Henry Pagalangan will also
monitor and evaluate OSH policies and programs under the OSH law.
So, dito naman sa emergency occupational health personnel and facilities ay merong two total
numbers of workers in cashier areas: 15 for onsite installation and maintenance and 5 for
administration. All departments have a first aider and one treatment room maliban sa onsite
installation.

In this section, we were talking about incident, accident, or injury reporting. This is important
mainly for the benefit of employees, as it provides useful records na tumutulong sa safety and
health committee para ma identify yung accidents or injuries para mas ma assess and mas ma
manage ng maayos yung risk sa workplace.

So, to assess the hazards, they provide essential Personal Protective Equipment tulad ng hard
hats, safety gloves, safety shoes, harnesses, and vests sa kanilang mga employee’s para ma
protect sa sila workplace hazards. And ito naman yung mga safety signages na na encountered
namin sa workplace or establishment.
