Module 4: Writing the Psychological Report
In an objective manner, try to assess yourself. Based on the information that you have learned
above, do you think you can write an objective psychological report? Why or why not?
Yes. We were given a task to write a psychological report as a requirement to our subject
Psychological Assessment. I was able to interview a client and collect data and information
that I’ll be needing for my report. At first, I was a bit nervous to conduct a face to face
interview with my client but at the same time I also feel excited since I feel like this is an
opportunity for me to see if I’m able to create a safe space for a person to share their
struggles with me and be comfortable to show their vulnerability with me. It’s important to be
respectful all the time even though they’re slowly being comfortable with you. Be patient
with your client because it’s important to respect their boundaries. I think that’s why I was
able to write a psychological report for the first time. I was able to collect information of my
client without offending them or making them uncomfortable. And once you finally got all
the data that you’ll be needing to write a psychological report, then rest assured that you’’ be
able to finish it.
What possible challenges do you think you might encounter as you write a psychological
report?
When you didn’t get enough data to write a psychological report. This happens when the
client was not able to fully express themselves and share their struggles. There are
circumstances when they can’t just drop all the information about their life. For example, a
client is not really interested to be interviewed in the first place, or they have trust issues and
can’t trust you with their personal information. The worse part of this is when the data or
information they have given you was a lie. It’d be a challenge to finish a psychological report
when that happens, that’s why it’s important that you’ll create a good atmosphere or safe
space for them where they can share their thoughts and feelings towards you.

Module 5: Practice Writing a Psychological Report
Referral Question
Juno is a 20 years old girl who experienced difficulty breathing and often stare blankly. She
says that she feels empty and frequently wakes up exhausted. She also reported that she
experienced difficulty concentrating and feelings of restlessness. Juno also said that she
enjoys the presence of her cousin whenever she visits her but once her cousin leaves, she
finds it difficult to adjust again and she will experience the feelings of emptiness and
loneliness.
Background Information
Family background and Personal history- Juno is the youngest in the family. Her father had
an extra marital affair and not open to the idea of mental health issues. She was really close to
her father until her father had an affair with the mother of her closest friend back in 2020. She
felt alone when her brother, who is really close to her went to manila for work. She has no
one to talk to ever since her brother left for work. Juno said she witnessed the issues her
parents tried to resolve silently. In most months of 2018, she reported that she would get
frightened by the loud voices of her parents when fighting in their bedroom. However, she
considered 2019 as a good year.
Medical History- Juno had a known history of depressive disorder and psychosis back in
2018. She has no notable deficit in cognitive faculty.
Current life situation- Juno is preoccupied by several things which includes her desires to
have an open communication with her parent and will need a strong support.
Behavioral Observations
She presented herself well in the clinic, dressed appropriately, looked neat and clean
Impression and Interpretations
Juno lacks support from her family causing her to feel helpless. She also relies to a single
person (her cousin) for emotional support since her brother wasn’t there to support her
anymore and talk to her. Her family problem was the main reason why she feels depressed
and constantly feel empty. Juno experiencing difficulty breathing and often stare blankly
could be the effect of depression and anxiety. It’s given that she’d feel exhausted and restless
because of this.
Summary and Recommendations
Having a healthy environment in a household and receiving strong support from Juno’s
family, especially her parents, could help her emotionally and mentally. It is recommended
that Juno should attend psychotherapy to help her develop new ways to look at things and
learn essential skills to challenge her negative beliefs through disputations and cognitive
reframing. This will help to reduce her depression and improve her functioning and social
interactions and increase positive effect. It is also advisable that she gets and receive an
emotional support from people around her especially from her parents.

Module 6: Psychological Assessment Process
Although it is the clinician who will really do all the phases in the clinical assessment
process, which among the phases in the framework do you think would be something that you
can do as a future psychometrician? In answering this question, please consider the
restrictions for psychometricians when performing clinical assessment and evaluation.
The phases in the framework that I think would be something that I can do as a future
psychometrician are phases 5 and 6. Psychometricians are in charge of developing, designing,
and implementing valid and reliable measures of psychological constructs. They also evaluate
the quality and accuracy of test scores, interpret and publish results, and train people on how
to use and interpret psychological exams. They also serve as psychological evaluation
consultants to corporations and individuals. The evaluation of an individual's level of
functioning, including their cognitive skills, emotions, and behavior, is one of the most
significant responsibilities shared by psychometricians and clinicians. Psychometricians are
responsible for administering tests as well as making sure that they are normed and
standardized correctly for the population being tested and that they are given and scored
consistently across test-takers.
The process of generating and administering psychometric tests requires a high level of
competence, as psychometricians must be skilled in areas such as test construction, test
administration, and scoring. Psychometricians must also choose and justify the use of
relevant tests, as well as evaluate and convey the outcomes of their assessments.

Module 8: Writing the Assessment Interview Report
Presenting Problem and Its History
Marielle Micaela N. Lozada is a 20-year-old college student pursuing a Bachelor of Science
in Psychology at PHINMA Rizal College of Laguna. She agreed to participate in this activity
because she wants to share her issues with someone other than her friends and get guidance
on how to deal with her traumas and problems without shutting herself down. Marielle claims
that she is much more comfortable expressing her personal difficulties with a complete
stranger because of her previous experience with a friend leaking her secrets to her
stepmother. She also states that she’s experiencing relapse from her traumas with her step
mom and exes.
Family Background
Marielle is a part of a broken family. Her parents are now separated and she’s now living
alone in her dorm. She has five siblings from her father’s side, and one sibling from her
mother’s side. Her mom is now planning to go to Canada. She receives support from her
mother but they’re not actually close with each other where she can share her struggles and
problems that she’s facing. Her father supports her financially and pays for her tuition and
lets her do anything she wants. Marielle was the first to find out that her father had an extra-
marital affair when she caught her father’s phone and saw the text messages between her
father and her step-mom. Her sibling from her step mom’s side was diagnosed with ASD or
Autism Spectrum Disorder, she states that it came from her step mom’s gene since her step
mom’s family have a history of ASD.
Personal History
Marielle experienced physical abuse and mental abuse with her past exes causing her to
suffer from relapse until now, but she’s now in a relationship with her boyfriend who gives
her emotional support. Marielle also states that she started feeling depressed and experienced
anxiety when she was in eleventh grade. She’d refuse to eat and come out of her room and
she’d cry for no apparent reason at all. That was also the time where she starts having suicidal
thoughts.
Marielle doesn’t have a good relationship with her step mother. They even got into a dispute
because her step mother accused her of leaking her father’s personal information, but in
reality, it was her step mom’s tactics to make her look like a bad daughter in her father’s eye.
Her step mom would even plant a chip and CCTV to monitor marielle’s actions and spy her.
This is also the reason why she’s paranoid most of the times that someone is following her
whenever she goes outside. But despite of this, marielle is fond of her little brother (who has
ASD) from her step mom’s side. Marielle said that ever since then, she already knew that
something was wrong with her little brother. At the age of 1, her little brother still can’t form
a word even the word “mama and dada” and can’t still walk. It took 4 years to take her little
brother to a professional doctor to check him and that’s where they found out that her little
brother has a level 1 ASD. Her step mom even argued that it wasn’t because of their genes
and the history of their family having ASD. After knowing that, marielle would willingly
tutor him to speak and read but her step mom was against it and told her to stay away from
her little brother which she did for the sake of peace.
Other Relevant Information
Marielle’s way of getting rid of her bad mood is through sleeping and eating food. She said
that she also like spoiling her friends whenever they go out. School is her way of escape to
forget all her worries and problems. She’s also learning how to get rid of her suicidal
thoughts but there are instances that even in her dreams, she’d see herself killing herself,
someone she knows kills themselves in front of her, and killing herself in front of her family.
She’d get nightmares like these whenever she feels happy, calm and relaxed. Marielle wanted
to seek professional help and is willing to look for a therapist, unfortunately, what’s holding
her back from doing it is because of how pricey it is. But it makes me feel at ease that she’s
willing to get better and willing to get a professional help.
