I Ehman Joy D. Palma a member of San Francisco Basketball Club under Madmax Retro
Garage Team would like to request that my schedule games of playing in basketball is only done
during weekends (Saturday and Sunday) because of unexpected Memorandum coming from our
Regional Office, I currently worked in the Bureau of Fire Protection (BFP) assigned at Prosperidad
Fire Station.

In connection to this, as per Rules and Regulations in the club that if a player will not play
during scheduled games, he shall impose penalty.

On this, May I humbly request the exemption of penalty during weekdays game schedules of
our team. Rest assured that I will play schedule games held during weekends (Saturday and Sunday).

Attached herewith is the Schedule for Game Practice and Regional Order as a Table Tennis
Player in Surigao City.
