MEDICAL REPORT OF EXEAT

RE: KOLAWOLE TIMOTHY AYOKUNLE / MALE /24 YEARS


I am writing on behalf of the above named prospective Corp member scheduled to serve in your State. The
above named person is a patient of our medical facilities. The patient is being managed for Retinal Detachment.
The patient suffered headache, blurred vision, photopia, seeing lot of floaters and darkening of side vision
whenever he is being attacked by the ailment, but he is being properly managed here in the hospital. Due
to his status, he has been advised to stay close to the hospital for proper medical attention by his
physician, and the patient has been scheduled for unavoidable surgery and the patient has put so much
trust in our doctors and has agreed to undergo the surgery at our hospital only with the support of his
family members.
Coupled with the stated above, he is also being managed on Chronic Peptic Ulceration. He suffered
abdominal pain, bloating and heartburn among others. He is currently on medications and has been
advised to stay away from rigorous activities. I recommend that he should be granted an Exeat for a
period of this 3 weeks to enable him to recover fully.
At this moment the patient has been fixed for weekly follow up at the hospital with his physician and he
has been advised to stay close to his family members to enhance his healing process.

necessary assistance he might enquire, Thanks
