Scope of this Product Comparison
This Product Comparison covers complete nurse call systems designed for routine or emergency communication between patients and nurses and other hospital staff. Excluded are systems used for patient management in exam rooms or in emergency rooms, systems used exclusively for paging and staff registry, and accessories such as dome lights and cordsets. Wandering or bed-exit alarms are not included but may be available with some nurse call systems listed in this report. station lights, low-voltage power supplies, and interconnecting wiring. The annunciator is usually located at the nursing station, which is often out of sight and hearing range of patient rooms. Signals can be initiated by patients or staff from any location with a room station, such as patients bedsides, bathrooms and showers, and utility rooms and other areas. Call devices come in various designs; many are wall mounted with push buttons and lights on the faceplate. Pendant-style switches have a pushbutton switch at the end of a cable (cordset) that plugs into a jack in the wall-mounted bedside station; some hospital beds have cordsets mounted on the siderails and provide adapters for interfacing with the nurse call system. Purpose
Nurse call systems allow patients and staff to summon hospital staff with visual and/or audible signals for routine or emergency needs. These systems can also summon emergency resuscitation teams; a system integrated with a staff locator system can, through the central station or an integrated telephone handset, indicate the location of staff within the hospital.

Principles of operation
Nurse call systems consist of five elements: a central console (annunciator), a remote switching device for each patient or patient room, corridor or intermediate

located in the patient pendant. Pneumatic cordsets house the electrical switch at the end of the cord that plugs into the wall socket of the nurse call system rather than in the patient pendant, thereby reducing the possibility of shock. The cord that runs from the patient pendant to the plug contains an air tube, and the pendant contains a push-button plunger, squeeze bulb, or pressure pad that, when pressed, transmits air pressure down the tube and closes the electrical switch at the opposite end. Puff/suck pneumatic signal tubing is often provided for incapacitated or paralyzed patients. Other call devices include disposable pull cords, which are often used in bathrooms, and systems with switches that allow the staff attendant to send a call response message, as well as the location of the call, to the central console. Many nurse call systems permit two-way conversations by means of a handset, headset, or speaker/ microphone at the nursing station and a speaker/ microphone at each call station. Call stations amplify patients voices; however, some patients may require cord-attached pillow speakers to facilitate communication. Many pillow speakers also provide controls for televisions and radios and may interrupt sound during a call. Corridor lights outside patient rooms and zone lights at corridor intersections are illuminated when calls are signaled to assist the staff in locating the calling room. The signal is relayed to the central console, triggering an audible tone and a display (e.g., numbered indicator lights; LCD, LED, or CRT screen) of the calling room number. Signals from bathrooms and showers trigger a tone and light signal (usually pulsating) different from the calls from beds because they may require more immediate attention. In addition to call prioritization, the central console usually displays the room number and status of the call. The central console may also track and display the length of time that a call has been in the system from the time the call is initiated until a room response is indicated. A call reminder feature alerts the attendant if a patient has not received attention within a predetermined amount of time, usually programmed by the hospital. Microprocessor-based nurse call systems central consoles can be preprogrammed to hospital specifications or can be user programmed, permitting staff to enter the name, room number, and medical data of each patient. These systems may interface with the hospital information system (HIS) and admission/discharge/transfer (ADT) system; they may also have an integrated staff-locator system staff manually press buttons at each room entrance or may wear an infrared transmitter badge, wristband, or similar device that interfaces with an infrared locator system to identify themselves. Infrared sensors are sometimes incorporated into the room stations or may be located in the ceiling or elsewhere in the structure of the hospital. They transmit identification information to the nurse call system and are most commonly used to locate staff, but they can also be used to locate patients or equipment. With an infrared locator system interface, the master station can provide a constant display of staff location. With appropriate software, the display can be altered to show the location of staff by level (e.g., registered nurse, housekeeping). The system may also have nurse follower capabilities the system constantly senses a staff members location and routes all calls to the location. The locator system can also be used to automatically cancel patient calls when a staff member of the appropriate level or one assigned to the patient enters the patient room. Some nurse call systems provide programmable code numbers, words, or red indicator lights (triggered when calls are signaled from priority patients or from bathrooms) next to the standard call lights on the display panel; emergency calls can be deactivated only from the room where they were activated. Other central consoles with limited display capabilities can present calls in a priority sequence, irrespective of the order of receipt. Patients can also indicate priority by signaling for a nurse or aide using labeled call buttons at the bedside station. Operating room personnel can initiate emergency signals with a footswitch, button, or other device to alert the nursing station or another department (e.g., anesthesia). Emergency rooms and intensive care units may have emergency buttons that are used to alert all staff to cardiopulmonary and other emergencies. Microprocessor-based nurse call systems can automatically route code calls to the assigned code team members via a radio pager or wireless telephone as soon as the code call is placed. Other nurse call systems display the code call at selected central locations (e.g., telephone operations).

Reported problems
The most common problem with nurse call systems is that cordsets at patient bedside stations break; they may also fail to signal the annunciator if they become disconnected. Breakage is typically caused by frequent use (especially those with television controls) and accidental dropping. However, cordsets are required by

Underwriters Laboratories (UL) standards to signal the annunciator if a disconnection occurs (although systems have failed to do so on occasion). Cordsets that generate electric arcs could be hazardous when used in or near oxygen-administering equipment such as oxygen tents. Cordsets that do not have switches in the patients signaling device, such as pneumatic cordsets and pull cords, can prevent this hazard. There are several problems associated with infrared locator systems. A caregivers transmitter badge may not be detected if the signal from the transmitter is physically blocked. Infrared systems may also have trouble locating staff when a transmitters battery is low. Some badges signal low battery strength, but they can be more expensive. The location and quantity of locater sensors is also critical to the effective operation of a locater system. Hospitals can interface nurse call systems with other equipment such as bed-exit detectors, physiologic patient monitors, ventilators, and infusion pumps. However, the nurse call system must be preconfigured to connect these devices and there may be limitations in distinguishing between each devices alarm. Some systems interface with ADT systems that are Health Level 7 (HL7) compliant. HL7 is defined by the seventh (applications) layer of the International Organization for Standardization-Open System Interconnect (ISO-OSI) standard. It is an electronic data interchange standard that addresses HIS functions, such as order entry, registration, ADT, billing, and results reporting, and clinical information systems. Cost containment Before purchasing a nurse call system, hospitals should be aware not only of the initial cost of the system, but also of the cost of supporting the system. These costs include servicing the system and training the hospital staff. If the system is microprocessor based, additional costs will include wiring the nursing stations to a network and upgrading software. With any system, the costs of site planning and installation should be considered. If the system replaces an existing nurse call system, the costs of disabling and removing the old system should also be considered. Full consideration of these costs will give the hospital a more realistic operating cost than the initial cost of the nurse call system.

Stage of development
Many nurse call systems have remained essentially the same for decades, signaling with simple flashing lights and audible tones. The addition of two-way audio communication between the patient room and the central station is common and in some cases mandatory. Incorporating microprocessor-based nurse call systems allows advancements through interfacing with infrared locating systems, the hospitals private branch exchange (PBX), information systems, and ADT systems. Direct links can also be provided to radio pagers and wireless telephones. Some systems also have management and quality control capabilities and can record and report call transactions and statistics.

Purchase considerations
In the United States, nurse call systems are required by various codes and for accreditation by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and are often included in the planning and design stage of new hospitals. The U.S. Occupational Safety and Health Administration (OSHA) requires that all nurse call systems be certified and/or approved by a nationally recognized testing laboratory (NRTL) such as UL. Hospitals must be aware of the requirements pertaining to each component before purchasing or modifying a nurse call system. Some U.S. states have specified their own requirements for nurse call systems. Hospitals should check with their states department of health for any regulations that may apply to their area. Systems vary in their degree of expandability. Those having only visual/audible signal displays can, under some circumstances, be modified for voice communication, provided the conduits are large enough for additional wiring. Many suppliers can custom design systems to interface with other communications systems (e.g., staff registries, pagers).

About the chart specifications


The following terms are used in the chart: Meets ANSI/UL 1069: Standard for hospital signaling and nurse call equipment (see Standards and Guidelines) in the United States. Capacity, beds: The maximum number of patient stations represented on the annunciator. Most systems can be expanded by wiring additional annunciators to the system. Self-diagnostics: A self-monitoring feature that displays circuitry malfunction. Distinct audible tones: Signals triggered at the central console when a call is received. Normal signals, which are constant or pulsating tones, can be canceled at the central console unless specified otherwise. Emergency signals, which are rapidly pulsating tones, can be canceled only at the calling room. Display capacity, beds: The maximum number of signals that the annunciator can display at one time. Hold/recall button: This allows the central console attendant to hold a call in the console while another call is answered by means of the handset microphone, and then return to the original call. Call reminder: Alerts attendant if a patient has not received attention within a predetermined amount of time. Priority sequencing: Calls are displayed according to preprogrammed priority levels. Staff register: An integral feature or separate module used to register available staff.
