B. Requirements
Application for Permit to establish and License to Operate Clinic (New):
1. Photocopy of the applicant's ID card (Owner/In charge/Director) or Letter
Temporary Stay Permit, for Foreign Citizens and if
licensing management is authorized to another person by attaching
power of attorney and photocopy of the power of attorney's ID card on sufficient stamp duty
with company stamp/stamp;
2. Photocopy of individual NPWP;
3. Photocopy of the Deed of Establishment of a Business Entity if the Primary/Main clinic is hospitalized

Application for a Clinic Operational Permit (Extension):

C. Validity Period
