Dear parent or guardian,

Your child has been invited to attend a field trip! We will be going to
Gurmat Singh on Wednesday, October 25, 2023.We will be leaving the
school at10:20 a.m. and will be returning at 12:30 p.m.

Please return the permission slip by THURSDAY, October 19.

Your signature below indicates your consent for your child to participate. IT
also indicates that you understand that if any injury occurs, the school will
make reasonable efforts to contact you. In the meantime, you give
permission, in the event of injury, that your student may receive emergency
medical aid, anesthesia, and/or operation if, in the opinion of the attending
physician, such treatment is medically necessary.

For your child to receive the maximum benefit from this field trip,
we will require 2 volunteers to help out . Please arrive at least 15
minutes before the scheduled time to receive instructions.
