Dear Parent or Guardian,

Your child is going on a field trip. Please read the information at the top of this form, then sign and return
the permission slip at the bottom of this form by November 22, 2013.
Field Trip Information:
Date: December 7, 2013
Location: ICE Frosty the Snowman, located at Opry Mills
Purpose: This field trip is a fun way to show real life application of some of the things weve been learning
in class, like simple machines, physics, and measurement.
Cost: $10 per student
Cash or check payable to: H.B. Williams Elementary School
Means of Transportation: School Bus
Leave school: 11:15 a.m. Arrive back at school: 3:15 p.m.
Special Instructions: A bag lunch will be provided for all of the students
