Questions Related to the RFP Process Request for Information

3. Copies of actual(s) for year- end financials for the last three (3) completed years for each facility Answer: Attached are the year-end financials that we have received for the last three fiscal years.

4. Complete budget line item detail for revenue and expenses for each facility under the RFP for the last three (3) years. Audited statements would be appreciated.
Answer: Attached are the audited financial statements for the last three fiscal years for the facilities. These do not have complete budget line item detail for revenue and expenses for each facility so we would refer you to the answer to question 3 for that information.

5. Copy of current Fiscal Year Operating Budget with year-end bottom line forecast. Line-item evidencing all revenue and expenses. Answer: Attached is the more recent report and the format that we receive it in.

7. Itemization of current insurance coverages in place (structural, liability, etc.).

Answer: Attached are the coverages provided to us by the existing vendor.
