The term ‘POKA-YOKE’ is derived from two Japanese words.
‘Poka’ means ‘Mistakes’ & ‘Yoke’ means ‘Avoid’.
Poka-Yokes are mechanism used to mistake-proof an entire process.
Its main objective is to achieve Zero Defects by preventing or
correcting mistakes as early as possible.
The term was adopted by Dr. SHIGEO SHINGO as part of the
Toyota Production System in 1960.
It was originally described as “Baka-Yoke” which means “fool-
proofing”.
Later the name was changed to “ Poka-Yoke” which means :
“mistake-proofing”.

Forgetfulness
Misunderstanding
Wrong Identification
Lack of Experience
Wilful (Ignoring rules or procedure)
Slow Process
Lack of Standardization
Surprise (Unexpected machine operation, etc)
Intentional (Sabotage)

Poka –Yokes deals with understanding why people make
errors and how to analyse the process to know where errors
are likely to occur and what root causes contribute to them .
