Background
• RPWD Act 2016, RPWD Rules under Section 100 (June 2017),
Disability Assessment Guidelines under Section 56 (Jan 2018).
• RPWD Act 2016,Chapter X ( sections 56-59) deals with certification.
• Section 58: (1)Any person with disability may apply in prescribed
format to certifying authority having jurisdiction.
• (2) Authority should assess as per guidelines under section 56 and
issue a disability certificate or written information on “No Specific
Disability”.
RPWD Act 2016 - Rules Notified in 2017
• Rule 17: Any person may apply (1)to the notified authority of district
of residence or (2) any competent authority of a government hospital
where PWD might have taken treatment in that connection.
• Requirements :
• 1. Form filled as per Form IV of this rules.
• 2. Residence Proof
• 3. Two recent Passport Photos.
• Aadhaar/ Aadhaar enrollment, if any ( Not Mandatory)
FORMS I-VIII
• Form I - III – Forms for PWD Employer’s Returns under section 13.
• Form IV: Application for obtaining COD for a PWD (Rule 17).
• Form V- COD (In cases of Amputation or Complete Permanent
Paralysis of limbs or Dwarfism and in case of Blindness).
• Form – VI COD (In cases of multiple disabilities).
• Form –VII - Form–VII COD (In cases other than those mentioned in
Forms V and VI) .
• Form VIII- Rejection with reasons with mention on appellate authority

What to do in OPD if a patient asks PWD
certificate
• Explain and encourage them to register and to apply at UDID portal.
• For faster processing they may approach to the hospital authorities
informing about their acknowledgment.
• A coordinating person can facilitate communication between board
members and applicants.
• All units/ departments may establish a SOP on processing
applications, if applicants are registered with them or if they attend
for evaluation on their OP day.

• Any one may seek for disability evaluation and it would be the boards
responsibility to dispose such applications within a period of 1 month.
• A coordinating person between MRD, Board appointments, specialists
and applicants is essential for smooth and faster processing.

• 1 or 2 dedicated computer data entry operators for the following
tasks.
• Sort out applications of patients treated at NIMHANS on daily basis.
• To update about number of applicants pending etc., details to the
board on a periodic basis
• To provide assessment appointment information to the clients.
• To help the specialist doctors and other board members for updating
assessment on regular basis.
• To generate reports and to maintain records offline in this regard.

• 1-2 members from MRD team for the following task assistance
• To verify the MRD records for its authenticity, if there is any uploaded
disability certificate issued by NIMHANS on regular basis.
• To provide relevant medical records to the assigned specialist and to
other board members at the time of disability assessment.
• To maintain separate medical records for those who have a bench
mark disability certificate for fast access to care.
