Background
• RPWD Act 2016, RPWD Rules under Section 100 (June 2017),
Disability Assessment Guidelines under Section 56 (Jan 2018).
• RPWD Act 2016,Chapter X ( sections 56-59) deals with certification.
• Section 58: (1)Any person with disability may apply in prescribed
format to certifying authority having jurisdiction.
• (2) Authority should assess as per guidelines under section 56 and
issue a disability certificate or written information on “No Specific
Disability”.
RPWD Act 2016 - Rules Notified in 2017
• Rule 17: Any person may apply (1)to the notified authority of district
of residence or (2) any competent authority of a government hospital
where PWD might have taken treatment in that connection.
• Requirements :
• 1. Form filled as per Form IV of this rules.
• 2. Residence Proof
• 3. Two recent Passport Photos.
• Aadhaar/ Aadhaar enrollment, if any ( Not Mandatory)
FORMS I-VIII
• Form I - III – Forms for PWD Employer’s Returns under section 13.
• Form IV: Application for obtaining COD for a PWD (Rule 17).
• Form V- COD (In cases of Amputation or Complete Permanent
Paralysis of limbs or Dwarfism and in case of Blindness).
• Form – VI COD (In cases of multiple disabilities).
• Form –VII - Form–VII COD (In cases other than those mentioned in
Forms V and VI) .
• Form VIII- Rejection with reasons with mention on appellate authority

Types of UDID Applications

• An Application for Verification and Digitization of existing valid


manually issued certificate under PWD 1995 or RPWD Act 2016.
• A fresh Application for Disability Assessment & Certification.
• Appeal against earlier Medical Authority Assessment Report.

medial authorities of that District ( As of Now).
• Authority can sort out applications based on type of disability, name,
enrollment number, date of application.

Few important issues
• 1 month time frame should be followed as it is the rule published by
Government of India.
• Onus of checking the details like name spelling, photo , whether the
address uploaded matches with the residence proof document etc.,
by medical board Chairman.
• Practical implementation challenges related to availability of specialist
doctors, board members on a date when appointment is fixed and the
need for disposing application within a month of receipt of
application.
New Applications for Assessment
• Staff of enrolled Hospital has to log in for checking the applications.
• Applicants can be those who have taken treatment/ not taken
treatment.( A specific mechanism and dedicated human resources)
• A Specialist/ Specialist doctor would assess based on fixed
appointment.
• Specialist Doctor assessment may be combined with Medical Board
assessment. ( A mechanism may be devised if separate)
• Medical Board Chair person has to update board recommendation
after verifying the data.

Intellectual disability
• 1. MS/CMO/CS
• 2. Pediatrician or Pediatric Neurologist (where available and for those
under 18 years of age) –
• If the age is above 18 years Psychiatrist or Physician
• 3. Clinical (or) Rehabilitation Psychologist.
• 4. Psychiatrist.
• For children with more than 5 age, this certificate should be renewed
at age of 5,10 and 18 years.
• For children under 5 years validity of this certificate is 3 years or until
5 years of age whichever is earlier
Mental Illness
• 1. MS/CS/CMO
• 2. Psychiatrist for clinical assessment
• 3. Trained Psychologist.
• No Clarity on the validity duration.
• Board has the discretion about validity and to state either temporary
with re assessment duration or as to declare as permeant disability
with no further reassessment.
Autism Spectrum Disorders

• 1. Clinical Psychologist (or)Rehabilitation Psychologist


• 2. A Psychiatrist
• 3. A Paediatrician or General Physician as the case may be.
• 4.MS/CS/CM/ Chair Person .
• The certificate would be valid for a period of 5 years for those whose
disability is temporary and are below the age 18 years .
• For those who acquire permanent disability, the validity can be shown
as “Permanent” in the certificate
Specific Learning Disability
• (1) The MS/CS/CMO or equivalent
• (2) Paediatrician or Paediatric Neurologist (where available)
• (3) Clinical or Rehabilitation Psychologist
• (4) Occupational therapist (or) Special Educator (or) Teacher trained
for assessment of SLD.
• Certification should be only for children above age 8 years.
• A repeat assessment should happen at the age of 14 and 18.
Certificate issued at the age of 18 is valid for life long
Disability due to Chronic neurological conditions
MS/PD
• 1. MS/CS/CMO
• 2.Paediatric doctor for childhood chronic neurological conditions /
Psychiatrist for mental illness due to chronic neurological conditions/
Neurologist for chronic neurological conditions without mental illness
• 3. Specialist for certifying locomotor disability
• (4) Trained psychologist (clinical or rehabilitation) to administer IQ
test
• Medical board is having discretion to opine on the validity of the
disability certificate.
Muscular dystrophy
• 1. MS or CS or CMO
• 2. Specialist in PMR (OR) Specialist in orthopedics
• 3. One specialist as nominated by MS as per the condition of the
person with disability (Another specialist may be the Neurologist)
Loco Motor Disability

What to do in OPD if a patient asks PWD
certificate
• Explain and encourage them to register and to apply at UDID portal.
• For faster processing they may approach to the hospital authorities
informing about their acknowledgment.
• A coordinating person can facilitate communication between board
members and applicants.
• All units/ departments may establish a SOP on processing
applications, if applicants are registered with them or if they attend
for evaluation on their OP day.

• Any one may seek for disability evaluation and it would be the boards
responsibility to dispose such applications within a period of 1 month.
• A coordinating person between MRD, Board appointments, specialists
and applicants is essential for smooth and faster processing.

• 1 or 2 dedicated computer data entry operators for the following
tasks.
• Sort out applications of patients treated at NIMHANS on daily basis.
• To update about number of applicants pending etc., details to the
board on a periodic basis
• To provide assessment appointment information to the clients.
• To help the specialist doctors and other board members for updating
assessment on regular basis.
• To generate reports and to maintain records offline in this regard.

• 1-2 members from MRD team for the following task assistance
• To verify the MRD records for its authenticity, if there is any uploaded
disability certificate issued by NIMHANS on regular basis.
• To provide relevant medical records to the assigned specialist and to
other board members at the time of disability assessment.
• To maintain separate medical records for those who have a bench
mark disability certificate for fast access to care.

Suggested pathway for already certified

• Client should upload on his own or with assistance from PSW of


treating team.
• NIMHANS may provide dedicated space and apparatus for scanning
and uploading certificates.
• Client should share the enrollment details to the computer operator/
PSW.
• Information should be provided to the specialist doctors who have
issued such certificate, for its verification online.
• MS may upload recommendation for UDID generation
