Nursing Diagnosis and Intervention
Nursing Diagnosis:
Fluid volume shortage related to vomiting and diarrhea
Nursing Intervention:
• Give patient his oral rehydration fluid prn.
• Maintain patient’s I. V line
• Give oral fluid every hour when vomiting stops.
Expected outcome:
Patient’s sunken eyes disappears while the wrinkle’s skin became normal
Nursing Diagnosis and Intervention
Nursing Diagnosis:
Altered bowel elimination pattern related to increase peristalsis and mass movement
Nursing Intervention:
• Serve bed pan properly as needed
• Give prescribed antibiotic like TCN (Tetracycline) 250mg 6 hourly
• Apply protective cream on the skin and use soft tissue to clean the arms.
Expected outcome:
The bowel motion reduced to 5 times per day
