Compounding is the act of preparing, mixing, assembling, packaging, and/or labeling a drug or device as the result of a practitioner’s prescription drug order or initiative based on the practitioner– patient–pharmacist relationship in the course of professional practice, or for the purpose of, or incident to, research, teaching, or chemical analysis and not for sale or dispensing. Compounding also includes the preparation of drugs or devices in anticipation of prescription drug orders, on the basis of routine, regularly observed prescribing patterns. Sebutkan tahapan2 dalam compounding 1. Preparatory 1. Judging the suitability of the prescription in terms of its safety and intended use and the dose for the patient. 2. Performing the calculations to determine the quantities of the ingredients needed. 3. Selecting the proper equipment and making sure it is clean. 4. Donning the proper attire and washing hands. 5. Cleaning the compounding area and the equipment, if necessary. 6. Assembling all the necessary materials and ingredients to compound and package the prescription. 2. Compounding 7. Compounding the prescription according to the formulary record or the prescription, using techniques according to the art and science of pharmacy. 3. Final Check 8. Checking, as indicated, the weight variation, adequacy of mixing, clarity, odor, color, consistency, and pH. 9. Entering the information in the compounding log. 10. Labeling the prescription. 4. Sign-Off 11. Signing and dating the prescription, affirming that all of the indicated procedures were carried out to ensure uniformity, identity, strength, quantity, and purity. 5. Cleanup 12. Cleaning and storing all equipment. 13. Cleaning the compounding area. Parts of a Prescription 1. Name of the Patient 2. Superscriptio merupakan kelengkapan dari resep dokter. Ditulis dengan simbol R/ (recipe= harap diambil). Biasanya juga sudah tercetak dalam blanko resep, terletak di sisi kiri atas, hanya tercetak satu R/ sehingga bila diberikan lebih dari satu BSO/ formula resep diperlukan penulisan R/ lagi. 3. Inscriptio merupakan inti resep dokter. Berisi nama obat, kekuatan dan jumlah obat yang diperlukan serta ditulis secara jelas. Penulisan nama obat dapat menggunakan nama generik, nama standar, atau nama paten. Penulisan jumlah dan kekuatan obat dalam satuan obat atau volume dengan sistem metrik (mg, g, ml, l). Penulisan jumlah obat dalam satuan biji (tablet, kapsul, botol, bungkus, supositoria, dll) dengan angka romawi (I,II,III, dst). 4. Subscriptio Bagian ini mencantumkan bentuk sediaan obat dan jumlahnya. Cara penulisan (dengan singkatan bahasa latin) tergantung dari macam formula resep yang digunakan. Jumlah obat ditulis dengan angka romawi (I,II,III, dst). 5. Signatura Bagian ini berisi informasi tentang aturan penggunaan obat untuk pasien, yaitu meliputi frekuensi, jumlah obat dan saat obat diminum, untuk setiap hari serta informasi lain yang mungkin diberikan simbol yang diberikan adalah S (signatura = tandailah). Walaupun aturan penggunaan obat oleh pasien sudah ditulis dalam resep, dokter berkewajiban menjelaskan secara lisan pada pasien saat menyerahkan resep. 6. Name of the Prescriber drg. Wina S.
