the same day and able to identify 10 closed
contacts including our index case.

 As company’s compliance to Department of

Health’s Guidelines on Covid-19
Management, Molex Integrated Products
Philippines, Inc. coordinated and scheduled a
swab test for the identified closed contacts
performed at Medical City Clark at Clarkfield,
Pampanga.

 Upon receiving a notification from the

company, stating that she was identified as
a closed contact, the index case initiated
home isolation and coordinated to the local
officials requesting for transport assistance.

 The Rural Health Unit II facilitated the

transport using the designated ambulance
of the municipality.

 The company notified and forwarded
the result of the swab test from Green
City Medical Center to the index case.

 shetested positive on RT-PCR for

Covid-19 and immediately notified the
Rural Health Unit II staff. The RHU
conducted an initial assessment and
data validation upon receiving the
report.
