1.0 Course Purpose

• This training course is designed to give new employees


a clear understanding of the Permit To Work system as
it is applied in TSP Production Operations (refer TSP Permit to
Work Procedure: TPS-P-PR-PRD-MET-001)

• It is important to note that staff training on making out and using


work permits is a continuous exercise

• Staff need to refer to this document which clearly defines the


procedure and the types of permit available.Note also that a
separate Procedure (PR-PRD-MET-002) covers offshore wellhead
platforms

• All personnel working on site must be familiar with


the procedure so everyone is aware of the risks and
can make their own contribution to overall safety

2.0 Permit to Work Procedure


The primary purpose of this Procedure is to ensure that written permission,
in the form of an approved Permit to Work, is correctly issued to allow authorised
personnel to perform clearly defined tasks
It is further to ensure that nothing is done, during the performance of these tasks, to
prejudice the safety of personnel or the installation
In particular, the following points must be observed:
•Correct operation of the Permit to Work (PTW) System is achieved
•All necessary personnel are aware of the safety precautions to be taken
•The equipment, or area affected by the work is clearly defined
•All departments concerned with the equipment, or area affected by the work, are
aware of its status and the progress of the work
•The correct protective clothing and other equipment specified on the PTW,
is provided and used
•Relevant Line Managers and Supervisors are kept aware and fully informed
of all work in progress

3.0 The Permit to Work

• The Onshore Permit to Work System is based around the use of Main Permits to
Work that cover all classes of work, whether carried out by Company
employees or Contractors

• Certain specific defined tasks also require Complementary Permits in addition


to the main Permits to Work

• Furthermore, certain routine tasks,


tasks which do not create
any potential hazard to personnel, equipment, or the site, will not require a
Permit to Work. Such tasks are listed
by the Process and Shift Superintendents/Supervisors for
the areas under their control. These listed tasks must be covered by detailed
Procedures to ensure safe performance and must have written approval from
the Operations Assistant, Phases 2 & 3

4.0 Types of Permit to Work

There are three, Main Permits to Work:


• Main Hot Work (Naked Flame) Permit
• Main Hot Work Permit
• Main Cold Work Permit

4.1 Main Hot Work (Naked Flame) Permit

A main Hot Work (Naked Flame) Permit is required for any work involving
equipment iving rise to a definite source of ignition, including, but not
limited to, use of equipment for oxy-acetylene burning, gas welding,
electric-arc welding, propane torch heating
and burning, grinding or use of a hot-air gun

This permit is identical to Main Hot Work Permit but clearly stamped
‘Naked Flame’

4.2 Main Hot Work Permit

A main Hot Work Permit is required for any work involving, or


giving rise to, a potential source of ignition, including, but not
limited to, grit blasting and work on non-intrinsically safe
equipment.
A Hot Work Permit is also required for electrical work, unless
the equipment is fully isolated

The Area Authorities and safety Authorities may refuse or


postpone Main Hot Work Permits, particularly the Main Hot Work
(Naked Flame) Permits, if they consider that the prevailing
conditions are not suitable for several simultaneous hot work
activities

4.3 Main Cold Work Permit

A main Cold Work Permit is required for all work that is not
covered by the Main Hot Work Permits and Main Hot Work
(Naked Flame) Permits described previously

5.0 Complementary Permits


In addition to the three Main Work Permits there are Nine,
Complementary Permits, that may be issued together with any
of the Main Permits

5.1 Entry to Vessels or confined Spaces

• This complementary Permit is required before any person


is allowed to enter an enclosed space for any purpose.

• An Enclosed Space is considered to be any area,


(including tanks, vessels, pipes, sumps, or pits), in which
there could be an accumulation of any flammable or
toxic materials and where gas, vapor, liquid or solid
residues may accumulate, or in which oxygen deficiency
may occur and which could prove harmful
to personnel entering without a suitable respiratory
protection device

5.2 Radiography
This complimentary Permit is required for any work entailing the
use of radioactive sources

5.3 Excavation
This Complementary Permit is required for any excavation work in
areas under TSP control

5.4 Process/Mechanical Isolation


This Complementary Permit is required to withdraw equipment from
service by physical isolation

5.5 High Voltage Electrical Isolation


This Complementary Permit is required for all work on electrical
systems or equipment where the voltage exceeds the following
limits:

5.5 High Voltage Electrical Isolation


This Complementary Permit is required for all work
on electrical systems or equipment where the voltage
exceeds the following limits:
• 1000 volts AC or 1500 volts DC between conductors
• 600 volts AC or 900 v0lts DC between conductors and
earth

5.6 High Voltage - Sanction to Test


This Complementary Permit is required to test the high
voltage electrical systems or equipment defined in 5.5
above

5.7 Low Voltage Electrical Isolation
This Complementary Permit is required for all work on electrical
systems or equipment where the voltage is lower than as defined in
5.5 above

5.8 Limitation of Access


This Complementary Permit is requiredto work
in any area close to live electrical systems or equipment. A Limitation
of Access Permit is not required where the work to be performed is
already covered by any of the other Main Permits to Work

5.9 Diving
This Complementary Permit is required to work on any underwater
equipment

6.0 Permit to Work Structure and Content


6.1 General
All the Permits to Work are four-part, uniquely numbered,
forms with a top White Copy, Green, Pink and Blue copies.
Each of the four pages has a large shadow-style numeral:
1,2,3 or 4, corresponding to the page number
Permit to Work forms all have a two-letter prefix identifier
as shown in the table:

6.2 Permit Structure

Although clearly there are many individual differences between


the various Permits to Work, there is a common overall structure
to enable the process to move smoothly. The four basic steps,
from start to finish, may be simplified and summarised as follows:

7.0 Responsible Persons
7.1 General
• All site personnel, without exception, have an individual
responsibility to ensure that the Permit to Work system operates
correctly and efficiently. The named positions
in column 1 of the following Table have further specific and
clearly defined responsibilities

• Not all of the named positions are TSP Job Titles, but are
Functional Titles for the purpose of this training course. Each of
the positions and their associated responsibilities are described
individually later in the course

• The table overleaf is arranged to show the correspondence


between the TSP Functional Titles,
the duties associated with those functions and the Functional
Titles as defined in HSE standard 601

8.0 Procedures
8.1 Administration of Permits
The Operations Assistant Phases 2 & 3 has the
ultimate and overall responsibility for the implementation
of the Permit to Work Procedure and also for limiting the
number of Permits that are valid at any one time
The routine administration of the Permit to Work
procedure is carried out by those persons designated
by the Operations Assistant. These designated persons
are responsible for:
•Issue of the Permits
•Management of the work progress
•Revalidation or cancellation of Permits

8.2 Procedures Applicable to All Permits


– The top (White) copy of all main Permits to Work and
any associated Supplementary Permits, are retained by
the person performing the work, or in charge of the group
performing the work. White copies of PTW’s must be clearly
displayed, and available for inspection at all times at the work sites
– It is the responsibility of all personnel involved in the work
to ensure that the conditions stated on the Permit are maintained
throughout the duration of the work

Every Main Permit to Work must:

• Clearly define the specific item of equipment to be worked on


• Show the precise location and exact nature of the work to be done
• List the number of persons in the working team

– All Permits are automatically suspended on the raising
of any Emergency Alarm. Only when the situation is back
to normal may the work be resumed. The Permit must be
endorsed to show the break in the work and the times
of the interruption
– During any interruptions, the work sites must be patrolled
by the Production Operators and Safety Authority
to ensure that all personnel are aware of the change
in conditions
8.3 Validity of Permits to Work
– Main Hot Work (Naked Flame) Permits and Main Hot Work
Permits are valid for a period of one shift (12 hours) only.
If work is to continue beyond one shift or the end of the
shift, whichever is sooner, a new Permit must be issued
– No Main Hot Work Permit can be used, or remain in use,
later than seven (7) shifts after the original PTW issue

– Main Cold Work Permits are valid for a maximum period of seven (7)
consecutive days, with revalidation required at every shift change. If for
any reason it is necessary to specify a shorter time limit, this is to be
noted in the ‘Valid Until’ section of the PTW
– The following three Complimentary Permits are valid for a period of one
shift (12 hours) only. If the work is to continue beyond
12 hours or the end of the shift, whichever is sooner, a new Permit must be
issued:
° Entry to Vessels or Confined Spaces
° Radiography
° Diving
– Complimentary Permits (High or Low Voltage Electrical Isolations) are
normally valid for the duration of the related Main Work Permit. However,
when the isolation is required for a longer duration than the Main Permit
(e.g. when equipment is isolated for a major repair outside the Site, or
when awaiting long term spare parts delivery), the Electrical
Complimentary Permits may remain valid
– Under these circumstances, the Complimentary Permit is logged as a Long
Term Isolation and remains in the Permit Office even though the
associated Main Permit has bee cancelled

8.4 Process/Mechanical Isolations


• Safety polypropylene straps with plastic tag plates must be
used at each of the specified mechanical locking-off points
where work is done under a Complimentary Permit
(Process/Mechanical Isolations
• The unique numbers of both the Main and the Complimentary
Permits must be clearly written on each
of the tag plates, using a suitable indelible marker.
On completion of the work and cancellation of the Permit, the
straps may be removed and the tag plates wiped clean
• Long Term Isolations require locking-off

8.5 Gas Tests


• The persons responsible for conducting and monitoring Gas
Tests are the Gas Tester and the Safety Authority. Their roles
and responsibilities are detailed in sub-sections 3.6 and 3.8

Gas testing is required for:

• All Naked Flame and other Hot Work activities


• Electrical work in designated Hazardous areas
• Entry into Vessels or Confined Spaces

• A dedicated section is provided on the reverse side of


the top white copy of each Main Hot Work (Naked Flame)
and Main Hot Work Permit to record the resulta of any
atmospheric sampling. This section must be completed
by a nominated Gas Tester prior to such work commencing.
Gas Tests must be repeated periodically throughout the
validity of the Permit

10.0 Summary
The purpose of this course was to describe the various types of
Permits available for issue under the Permit to Work System for
the South Pars Phases 2 & 3 onshore facilities, plus any other
facilities to be operated by TSP. It has identified the persons
responsible for the issue and use of
the Permits and the circumstances under which they should be
used
