    This document describes the treatment of a 19-year-old male patient with a unilateral cleft lip and palate using distraction osteogenesis of the maxilla, Lefort I osteotomy, and bilateral sagittal split osteotomy (BSSO). The patient presented with a hypoplastic maxilla, prognathic mandible, and asymmetry. Treatment objectives were to correct the skeletal bases and crossbite. Treatment involved initial leveling, maxillary expansion, extraction of teeth, decompensation, maxillary distraction osteogenesis, and BSSO to setback the mandible. Post-treatment imaging showed significant improvement of the skeletal bases and soft tissues.
