12-year-old boy is brought to the physician because of a 2-day history of
nausea, abdominal discomfort, and yellow discoloration of the eyes. Six
weeks ago, he had an episode of fever, joint pain, swollen lymph nodes,
and an itchy rash on his trunk and extremities that persisted for 1 to 2
days. He returned from a backpacking trip to Colombia two months
ago. His temperature is 39°C (101.8°F). Physical examination shows
scleral icterus. Laboratory studies shows elevated titres of hepatitis b
surface antigen. What is the most likely cause of this patient's findings?
Viral hepatitis is a systemic disease characterized by a predominant lesion of the
immune system, liver, digestive tract and clinically manifested by intoxication,
hepatomegaly, jaundice.

Hepatitis B is an acute liver disease characterized by an acute onset of the disease,
varying degrees of severity, from asymptomatic to fulminant forms, long course,
with the possible development of chronic hepatitis  and cirrhosis of the liver .

The causative agent of the disease -


DNA-containing virus, belongs to the
family of hepadnoviruses. It has a
spherical shape, consists of a nucleocapsid
and an outer shell. In the center of the
nucleocapsid is the genome of the virus,
represented by two strands of DNA. The
virus contains from antigens: HB s -
antigen, HBs-antigen (nuclear, cow) and
HBe-antigen (antigen of infectivity)

PHASES OF HBV INFECTION
 Early isolation of ill person.
 Sterilization of instrument.
PROPHYLAXI  Passive prophylaxis by human immune globulins.
S  For hepatitis B active prophylaxis: after the birth in 1,6
months. When mother is HBsAg positive-after the
birth in 1,2,12 months.
