• It is a written order by a registered medical practitioner to a
pharmacist intending the ingredients (drugs) he would like to
administer to his patient with clear instruction to the pharmacist for
its preparation and dispensing and a very clear instruction to the
patient for its use.

• Rx
• Maintain airway – lateral decubitus, Nasal trumpets, O2 suction
• Inj. Dextrose 5% IV (hypoglycemia)
• Inj. Lorazepam (0.1 mg/kg over 30 - 60 seconds upto 6 mg) IV, may repeat in
5 mins.
• IV midazolam (0.5 – 0.3 mg/kg) or IM midazolam (0.2 mg/kg upto 10 mg)
• Inj. Levetiracetam 1 gm IV over 15 – 20 min
• Inj. Sod. Valproate 1 gm IV over 15 – 20 min
• Inj. Phenytoin 1 gm IV over 15 – 20 min
• Inj. Propofol 2mg/ kg loading dose over 5 min, then infusion 10 mg/kg/hr
• Inj. Pentobarbital 5 mg/ kg over 10 min, then 1- 5 mg/kg/hr for 24 hr.

• Maintain airway, O2,Propt up position
• Nebulization with salbutamol 2.5 mg + Ipratropium Bromide 0.5 mg
intermittent inhalation driven by O2
• Inj. Hydrocortisone hemisuccinate 100 mg i.v. stat, followed by 100 - 200 mg
4-8 hourly infusion: may take upto 6 hours to act.
• Inj. Aminophylline 250-500 mg diluted in 20-50 ml glucose (5%) solution IV
over 20 - 30 min
• Inj MgSO4 2 gm stat
• Intubation and mechanical ventilation, if needed.
• Inj. Sodium Bicarbonate IV

• 3rd dose – 300 mg base (5mg/kg) (2 tab) after 24 hours
