Adverse weather like, wind speed, wave,
current,OSV along side…
Defective lifting equips like Wire sling
crack or worn out, shackles bending or worn
out, Stinger hook corrosion or worn out and
crack found…
Overload, standing under load,
miscommunication due to radio failure or not
good visual.
Not following manufacture SWL of
manufacturer and poor maintenance

Flying objects during transfer.
Hand pinch points
Narrow and obstruction area
Slip trip fall.
Defective tug line
Lifting gear and lifting equipment not to be
checked and inspection before lifting.
Not follow routine maintenance as per AMOS
requirement.
 4. Method to control the Hazards
 Work under Policy, procedure, PTW, JHA,
toolbox talk.
 Following Procedure (PR-MDK-PR21& PR-
MDK-PR24)
 Full PPE must be worn.
 Inspection and visual check lifting gear carefully
before use. (color code, SWL, condition and valid
lifting equips certificate.
 4. Method to control the Hazards
Sufficient man power (signal man, tag line man)
Lifting gear and lifting equipment must be
checked and test follow checklist require to make
sure all the step strictly follow.
All lifting gear must be routine maintenance as
per requirement in AMOS.
Good communication between crane operator
and signalman.
Radio must be checked and tested before job.
4. Method control the Hazards

 Use the tag line for reduce the load movement

 Barricaded lifting area and warning sign
 Escape route clear and beware of the lift
movement.
 Never stay below the lift.
Check the lifting area to make sure no obstruction
and slippery to avoid slip strip and fall.
Be careful with the hand to avoid the pinch point.

 5. Maintenances and Storage lifting equips
Daily: Visual inspection before use ( SWL, Color
code, any damage or defective).
Monthly: Inventory and maintenance.
06 Monthly: inspection, change color code,MPI and
load tested and issue certificate by authorized third
party. (for lifting gear like: webbing, round,
wire/sling, shackles…

03 yearly: MPI and load test by third party for all
Monorails, Provision crane, Over head crane…
05 yearly: MPI and load test by third party for Hose
handling crane, Pedestal crane
Note: Some equipment must be carried out
load test by third party after overhaul, big
repaired and replacement.

Recovery the Billy Pugh basket after used.

 Secured crane on the boom rest after used

Check the condition and cleaning then return back to
the authorized person (L.GPO) with signature after
used.
If found any damage of failure of lifting gear
equipment the user must be stopped immediately
 No use any unknown lifting gear and isolation one
(red color).
