Abstract

“Health is a state of complete physical, mental, and social well-

being and not merely the absence of disease or infirmity.”

We all have visited doctor atleast once. Having a regular physician means
having someone who knows your medical history and who will work with you
to shape your healthy present and future.

What if you recently moved out of the town and you are in a situation that you
cannot consult your regular physician?It will be a hard for patients to express
all their previous medical records and symptoms to a new doctor.

With this facility we ensure that the the communication gap between doctor and
patient will be erased as doctor will be provided with all the necessary
previous reports to consult a patient
Existing system

In existing system patient consults doctor and doctor enquires of present

symptoms and check for their previous symptoms mostly based on the
words of patients itself. And based on it doctor makes a note on it and
writes out the medicines for the patient in paper.
 Problem Statement

→ To consult a doctor we should take all the previous medical records. As

those are paper it would be worn and torn.

→ Expressing previous health issues to a new physician is harder.

→ Time consuming for doctors to write medicine note on paper.

→ Patients have to take scan reports again if the previous is lost

→ Consuming paper (as crores of patients consult doctor on a single day

crores of paper are wasted)
