FB group chat immediately using the prescribed damage
form for immediate field validation. Municipalities not
using the prescribed form will not accepted.

 Signed damage report (hardcopy) will be submitted within 1

month after the occurence of damage. Damage report
submitted beyond 1 month will not be accepted.
 Update planting report regularly. Unmatch figure of
planting report area to damage reported area will not be
honored.
