■ Vaginal Canal
– As a general rule, the vaginal canal of a virgin is tight
and the repositories are sharp and prominent
■ Wall of vagina is composed of smooth muscle &
fibroelastic connective tissue so that its tightnesss and
degree of resistance on insertion or an instrument depends
on the integrity of its wall and potency of its lubricating
secretion
– Sharpness of the wall’s repositories may be
diminished by insertion of foreign bodies, passage of
clotted blood, self-manipulation etc. and not by sexual
intercourse

■ Hymen
– Physicians give importance to the exam
of hymen in the determination of
virginity
– Classification of Hymen
■ As to Shape & Size of Opening
■ As to Structure & Consistency
■ As to Number of Opening
Hymen
■ As to Shape & Size of Opening
– Annular or circular
– Infantile
– Semilunar or crescenteric
– Linear Cribriform
– Stellate
– Septate
– Fimbriated
– Imperforate

Hymen:
■ As to Structure & Consistency
– Firm with strong connective
tissue and plenty of blood
vessels
– Thick yielding hymen with
scarce blood vessels
– Membranous hymen
Hymen:
■ As to Number of Opening
– Single orifice
– Septate
– Multiple
– Imperforate
Parts of the Female Body in the
Determination of the Condition of
Virginity
■ Labia Majora and Labia Minora
– Condition of either one not a reliable basis to determine
virginity
– Condition of the labia is much more related to general
physical condition of the woman rather history of previous
sexual intercourse
■ Stout female can preserve the plump, coaptated and firm
labia
■ Skinny female – usually have gaping labia
Parts of the Female Body in the
Determination of the Condition of
Virginity
■ Fourchette
– Presents a v-shaped appearance as the 2 labia
minora unite posteriorly
– May become rounded with retraction of the edges.
– Other than SI stretching may be caused by
■ Stretching apart of the thighs
■ Horseback or bicycle riding
■ Instrumentation
■ Breast
– Condition not reliable to determine virginity
Parts of the Female Body in the
Determination of the Condition of
Virginity
■ Breast
– Condition not reliable to determine virginity
– Size, shape & consistency may be hormonal or
hereditary
– A fully developed breast may be classified as to shape
■ Hemispherical
■ Conical
■ Infantile or flat
■ Pendulous
– Hemispherical pendulous
– Conical pendulous
DEFLORATION

■ Defloration
– Laceration or rupture of the hymen
as a result of sexual intercourse.
■ all other lacerations of the hymen
which are not caused by sexual
intercourse are not considered as
defloration.
Parts of Female Genitalia that must be
Examined to Determine Defloration

■ Vaginal Canal
■ Hymen
■ Condition of the vulva
■ Fourchette
Parts of Female Genitalia that must be
Examined to Determine Defloration

■ Vaginal Canal
– After repeated sexual acts, there may be
diminution of sharpness or obliteration of
vaginal rugosities
– Vaginal wall, together with vulva may
suffer injury during defloration or some
other causes
Predisposing Causes of Vulvo-
vaginal Injuries During Sexual Act
■ Virginity
■ Prepuberty
■ Genital Disproportion
■ Unprepared or unaroused female
■ Position during sexual act
■ Brutality of the male partner during sexual act
■ Recent vaginal surgery
■ Excessive active involvement of female partner
■ Multiple sexual act among sex deviants
■ Renewed sexual activity after prolonged abstinence
■ Post-menopause
■ Uterine retroversion
Parts of Female Genitalia that must be
Examined to Determine Defloration

■ Hymen
– Hymen can be lacerated during
initial sexual act but not always the
case
■ If ‘hymen is intact’ does not prove
absence of previous sexual intercourse
& the presence of laceration does not
prove defloration
Findings to be included in the Medical
Exam of Hymen

■ General condition of the hymen

■ Original shape of the orifice
■ If lacerated, ff to be noted
1. Degree of laceration
■ Incomplete laceration
– Superficial
– Deep
■ Complete
■ Compound or complicated
Findings to be included in the Medical
Exam of Hymen

■ If lacerated, ff to be noted
2. Location of Laceration
3. Duration of the laceration
■ Fresh bleeding laceration
■ Fresh healing with fibrin formation & edema of
surrounding tissue
■ Healed laceration with congested edges and with sharp
coaptible borders, aka recently-healed laceration
■ Healed lacerations with sharp coaptible borders without
congestion
■ Healed laceration with rounded non-coaptible borders and
retraction of the edges
Findings to be included in the Medical
Exam of Hymen

■ If lacerated, ff to be noted
4. Complications of laceration
■ Secondary infection
■ Hemorrhage
■ Fistula formation
■ Stricture
■ Sterility
