BAASTRUP SYNDROME

(Kissing Spine)

INTRODUCTIO
N
• Baastrup’s disease, also known as kissing
spine syndrome and interspinous bursitis,
results from adjacent spinous processes in
the lumbar spine rubbing against each other
and resulting in a degenerative and
inflammatory changes.
• It was first described in 1933.
• Kissing Spine mainly affects the lumbar spine
L4-L5 area.
ETIOLOGY & RISK
FACTORS
• Age is the most important factor
• Excessive lordosis which results in increased mechanical pressure
• Repetitive strains of the interspinous ligament with subsequent
degeneration and collapse
• Incorrect posture
• Traumatic injuries.
• Tuberculous spondylitis
• Bilateral forms of congenital hip dislocation
• Obesity
CLINICAL
FEATURES
• Excessive lordosis (excessive inward curvature of
spine)
• Back pain more specifically, midline pain , that
radiates distally and proximally, increasing on
extension and reducing on flexion
• Rotation and lateral flexion are very painful.
DIAGNOSIS OF BAASTRUP

• PHYSICAL EXAMINATION*
• The physiotherapist uses active and passive techniques with the
intention of evoking complaints.
• Active spinal extension can reproduce the symptoms.
• STORK TEST is very beneficial in the examination of this disease.
STORK TEST:- Patients positions The patient
stands on one leg and then extandes the
spine.this position is repeated with the other
leg.
TREATMENT OF BAASTRUP
SYNDROME
• Non Surgical Treatment:
• Steroid injections: To reduce pain and inflammation at the affected area.
• During this treatment period, extension movements of the lumbar spine
should be avoided.

SURGICAL TREATMENT
Osteotomy: A wedge-shaped cut into bone to correct alignment.
• Partial or total removal of an affected.

• PHYSIOTHERAPY MANAGEMENT
• The main goal is the reduction of pain and to improve spinal function.
• Strengthening and stretching of the abdominal and spinal muscles.
• Reduce excessive lordosis.
• Postural education and Hip flexor stretch.
• Other treatment Such as heat Therapy and muscles relaxation techniques can be
