RHEUMATIC

HEART DISEASE
RHEUMATIC
FEVER
Acute immunologically
mediated multisystem
inflammatory disease

That occurs few weeks

after an episode of group A
streptococcal pharyngitis
PATHOGENESIS
Hypersensitivity reaction

Cross reaction of antibodies against

streptococcal M protein with glycoprotein
antigens in heart, joints and other tissues

The M proteins molecules has a particular

surface exposed antigenic domain against
which Rheumatic fever patients mount a
strong antibody response

Chronic Valve leaflets are involved and characterized by-

Rheumatic • Leaflet thickening

• Commisural fusion

carditis • Bridging across valvular commissures create “fish

mouth” or “Buttonhole” stenosis
• Shortening, thickening and fusion of chordae tendinae

Most commonly involved valve- Mitral valve

Irregular thickening of the posterior wall of left

atrium

MacCallum patch

during acute rheumatic fever
