Healthcare Decisions Day is a project
HEALTHCARE started in the USA (2008) used to inspire,
DECISIONS educate and empower the public and
DAY providers (healthcare providers) about
AWARENESS the importance of advance health care
planning.

As early as 2006, I have made a formal
declaration of my wishes on how I am going to

should know about starting this conversation with your loved ones.
HEALTHCARE 1. You can start out by writing a letter—to yourself, a loved one, or a

think about producing for the Advance Directive.
Five Wishes is a document that, like other
advance directives, makes your wishes known
HEALTHCARE ahead of time. It is unique among other advance
DECISIONS directives, however, because it addresses all of a

that I am very sick and am not able to speak for
myself, I want the following wishes, and any
other directions I have given to my Health Care
Agent, to be respected and followed.
What You Should Keep in Mind as my Caregiver
HEALTHCARE • I do not want to be in pain. I want my doctor to
DECISIONS give me enough medicine to relieve my pain,
DAY even if that means that I will be drowsy or sleep
