Definition

Intraductal carcinoma of prostate (IDCP)


Is high grade prostatic adenocarcinoma
involving duc or acinar structures.

•IDCP is associated with high stage disease
with a significant negatif impact on cancer –
specific survival
• accurate diagnosis is crucial to ensure
appropriate patient management
Recomended by 2016 WHO classification,
suggests that the classification of IDCP with
micropapillary or loose cribriform architecture
without comedonecrosis should have cells > 6x
nuclear enlargement.
To evaluate the extent of nuclear
enlargement :

•Morphometric analysis related to each of these


parameters in 100 radical prostatectomy
specimens.

•Examines 100 Nuclei from foci of IDCP and


50 nuclei from foci of normal luminal epithelium
each patient
How to diagnose IDCP ?
Based on cells with definite features of
carcinoma present within duct/acinar
structures.
The parameter between IDCP cells
and benign luminal cels

Statisticaly significant enlargement


in nuclear perimeter, nuclear area and nuclear
diameter (P<0.0005).
The difference in mean of nuclear size:
was limited to factors of 1.3x, 1.6x, and 1.3x
respectively
There are three patients had rare
large nuclei:
If compared with benign nuclei size:

Nuclear perimeter :2.0x, 2.1x, and 2.1x.


Nuclear are : 3.6x, 3.8x, and 3.8x.
Nuclear diameter : 3.0x, 2.5x, and 2.5x

The definiton of nuclear enlargement of > 6x


was not reached in this cases.
In these cases  features of duct
invasive carcinoma.
So, the reliance on nuclear size criteria
would have resulted in underdiagnosis of
IDCP.
