The patient underwent surgery under spinal anesthesia to repair a grade IV perineal rupture sustained during childbirth. The surgical team sutured the rectal mucosa, anal sphincters, and perineal tissue in a layered technique using vicryl sutures. Following closure of the surgical site, the patient's condition was stable.

• The patient was laid on the operation table with iv line installed well • Under Spinal anesthesia.The patient was positioned in a lithotomy position and then performed aseptic and antiseptic measures with povidone iodine • The surgical field was closed with a sterile drape • evaluation of bleeding was carried out, concluded with a grade IV perineal laceration • Suturing the rectal mucosa with vicryl 3.0 was technique simply interrupted • The anal sphincters were sutured overlapping with vicryl 2.0 • rectal mucosal was sutured continuous layer-by-layer suturing is performed. • Then the bleeding is evaluated, the impression is controlled. • wound is closed with sterile gauze • The general condition of the patient after surgery is stable
