“ AN ORDINANCE MANDATING THE ACCREDITATION CODE OF CIVIL
SOCIETY ORGANIZATIONS FOR REPRESENTATION IN THE LOCAL SPECIAL
BODIES OF THE PROVINCE OF CEBU”

ACCREDITATION REQUIREMENTS
Pursuant to Provincial Ordinance No. 2021-03

TYPES OF APPLICATION:
1. NEW APPLICATION
(ARTICLE V, General Accreditation Requirements for New Application
[GARNA])

2. APPLICATION RENEWAL
(ARTICLE VI, General Accreditation Requirements for Renewal of
Application [GARRA])
ACCREDITATION REQUIREMENTS
Pursuant to Provincial Ordinance No. 2021-03

NEW APPLICATION
(ARTICLE V, General Accreditation Requirements for New Application [GARNA])

a. LETTER OF INTENT – must state and specify CSO’s intention to be accredited with the Sangguniang Panlalawigan of Cebu for the
purpose of representation in the Local Special Bodies;
b. APPLICATION FOR ACCREDITATION – must bear the official indicia/logo or letterhead of the CSO;
c. BOARD RESOLUTION/MINUTES OF THE MEETING – state intention for the Application with the SP for the purpose of
representation in the Local Special Bodies;
d. ARTICLES OF INCORPORATION/COOPERATION OR REGISTRATION CERTIFICATE (issued by any National Government Agency
that is empowered by law or policy to Accredit or Register organizations, groups, or associations such as:

DEPARTMENT OF LABOR AND EMPLOYMENT (DOLE) DEPARTMENT OF AGRICULTURE (DA)

DEPARTMENT OF SOCIAL WELFARE & DEVLPT (DSWD) DEPARTMENT OF HEALTH (DOH)
NATIONAL ANTI-POVERTY COMMISION (NAPC) INSURANCE COMMISION (IC)
PROFESSIONAL REGULATORY COMMISION (PRC) NATIONAL COMISSION ON INDIGENOUS PEOPLE (NCIP)
ACCREDITATION REQUIREMENTS
Pursuant to Provincial Ordinance No. 2021-03

NEW APPLICATION
(ARTICLE V, General Accreditation Requirements for New Application [GARNA])

e. CERTIFICATE OF GOOD STANDING / EQUIVALENT CERTIFICATION - varies depending on the CSO’s Registering Agency

- if with SEC - Certificate of No Deregatory Record - if with CDA - Certificate of Compliance


- if with DOLE - Certification stating that the organization or group has submitted its Annual Report under a
specific Department Order for a particular Calendar Year
- OTHERS - other similar certifications with the same effect/tenor as long as the said certification expressly
state relevant information as to the general standing of the concerned organization, association or group;
f. VERIFIED & VALIDATED LIST OF CURRENT OFFICERS & MEMBERS - must be signed by the Secretary;

g. ORIGINAL SWORN STATEMENT - indicating that the Organization is NON-POLITICAL & NON-PARTISAN;

h. BARANGAY CERTIFICATION - certifying the Principal Address of the CSO and that the CSO is holding its principal office address in
the said barangay

APPLICATION RENEWAL
(ARTICLE VI, General Accreditation Requirements for Renewal of Application
[GARRA])

a. LETTER OF INTENT – must state and specify CSO’s intention to be accredited with the Sangguniang Panlalawigan of Cebu for the

i . FINANCIAL STATEMENT or INDEPENDENT AUDITOR’S REPORT


- must be signed by the President, Treasurer and Auditor UNDER OATH; and
- must be forthe IMMEDIATELY PRECEDING YEAR from the Date of Application
g. MINUTES OF ANNUAL MEETINGS OF THE IMMEDIATELY PRECEEDING YEAR AS CERTIFIED BY THE BOARD SECRETARY;

h. ACCOMPLISHMENT REPORTS - for the immediately preceeding year of operations from the date of application.

VALIDITY

“The validity of the accreditation of Civil Society Organizations SHALL BE COTERMINOUS


WITH THE LOCAL CHIEF EXECUTIVE concerned. Accreditations made after the selection
period for representation in the Local Special Bodies, shall also be coterminous with the
local chief executive. “ (Sec 1)
ARTICLE XI of PROVINCIAL ORDINANCE 2021-03
Grounds for Revocation of Certificate of Accreditation

a. MISREPRESENTATION IN, OR FALSIFICATION OF, ANY DOCUMENT SUBMITTED IN SUPPORT OF


THE APPLICATION OF THE CSO;

b. FAILURE BY THE CSO DURING THE VALIDITY PERIOD OF THE CERTIFICATE OF ACCREDITATION TO
FAITHFULLY SUBMIT YEARLY THEIR FINANCIAL STATEMENT AND ACCOMPLISHMENT REPORT;
c. VIOLATION BY THE CSO DURING THE VALIDITY PERIOD OF THE CERTIFICATE OF ACCREDITATION,
OF ANY LAW, RULES AND REGULATION IN RELATION TO ITS OPERATIONS AND RELATIONS TO THE
PUBLIC;
d. REVOCATION, CANCELLATION AND EXPIRATION OF THE CERTIFICATE OF REGISTRATION OF THE
CSO FROM ITS REGISTERING AGENCY;
e. NON-COMPLIANCE OF REPORTORIAL REQUIREMENTS AS MANDATED BY LAW OR ORDINANCE;

f. HABITUAL ABSENTEEISM OF THE REPRESENTATIVES OF THE CSO IN GIVEN LOCAL SPECIAL BODY
OF THE PROVINCE OF CEBU;
