Root Cause Analysis
Root cause analysis (RCA) is the process of
discovering the root causes of problems in order to
identify appropriate solutions.

RCA assumes that it is much more effective to


systematically prevent and solve for underlying issues
rather than just treating ad hoc symptoms and putting
out fires.

Fish Bone Diagram

A fishbone diagram, also called an Ishikawa


diagram, is a visual method for root cause
analysis that organizes cause-and-effect
relationships into categories..
While the categories can change, the most
commonly used are :
1. Man
2. Machine

5. Measurement

Affinity Diagram
An Affinity Diagram is a tool that gathers large
amounts of language data (ideas,
opinions, issues) and organizes them into
groupings based on their natural
relationships (Viewgraph 1)

Step 3 - Sort ideas into groups
Step 4 - Create header cards

Interrelationship Diagram
An interrelationship diagram (ID) shows how
different issues are related to one another.
It helps identify which issues are causing Faktor 1
problems and which are an outcome of other
actions. It also shows the strength of each Critical
influence.
