 Introduction
• No matter how hard we try, every machine
will eventually breakdown!
• How we handle these breakdowns when they
do occur is indicative of the length of the
downtime that will obviously ensue.

• An appropriate mill breakdown report will

facilitate a smart and effective intervention.
 What a Repair Request Form is

• A repair request form is the document which

will be used to get a repair service approval
from the Mill Manager.
• It is otherwise called a “Job Order.”
 Importance of Repair Request Forms

• It assists in organizing, tracking, and keeping

of service request records.
• Without these forms, the Mill Manager will
have a hard time monitoring the functioning
of the mill and it makes it difficult for him/her
to come up with financial reports and budgets
at year end.
