Preparatory Activities prior to submission of Application
for DOH Certification
PREPARATORY ACTIVITIES
The facility shall internally assess its program
implementation following the certification
standards written in the SAF.
Phases:
1. Facility Assessment Phase – the facility
staff undertakes its assessment based on the
standards written in the SAF.
2. Mentoring Phase – the TA provider guides
and assists the facility in fulfilling the
standards for DOH Certification and ensures
the proper filling out of SAF to facilitate their
application for DOH Certification.

⮚ The facility shall undergo self-assessment and fill out Facility / TA
of SAF, which will serve as the initial SAF. Provider
⮚ The facility shall request TA through letter, text, or
call to Development Management Officers or
Provincial/City Coordinator. The TA provider shall use
the accomplished initial SAF of the facility to
document the findings and recommendations while
assisting the facility in fulfilling the standards for DOH
Certification.
⮚ Once all standards are met based on the
assessment of the facility staff and TA provider, they
shall accomplish the final SAF with a note for
endorsement for DOH Certification and signature of
TA provider.
