ERISIPELA
 La erisipela es una enfermedad infecciosa bacteriana que
afecta a la piel y al tejido subcutáneo, especialmente del
rostro, y se caracteriza por la aparición de placas rojizas.
Frecuentemente está causada por una bacteria llamada
estreptococos y casi siempre va asociada a episodios de
fiebre.
FISIOPATOLOGIA
 Las bacterias entran en la barrera cutánea afectada (e.g.,
úlcera, ampolla, mordedura de perro).

 La invasión de la dermis superior produce eritema y aumento


de volumen local de la piel (edema).

 Propagación de agentes bacterianos y citocinas inflamatorias


a través de vasos linfáticos superficiales.

 La erisipela generalmente es causada por las bacterias


estreptococos del grupo A.
FACTORES PREDISPONENTES

 Enfermedades de fondo (concomitantes): micosis del pie, diabetes mellitus,


obesidad, insuficiencia venosa crónica insuficiencia crónica (adquirida o
congénita) de vasos linfáticos eccema, etc.

 Presencia de focos de infección crónica por estreptococos: amigdalitis,


otitis, sinusitis, caries, enfermedad periodontal, osteomielitis,
tromboflebitis, úlceras tróficas (más a menudo con las extremidades
inferiores).

 Riesgos ocupacionales asociados con mayor traumatización, contaminación


de la piel, uso de zapatos de goma, etc.

 Enfermedades somáticas crónicas, por lo que la inmunidad anti-infecciosa


disminuye (más a menudo en la vejez)
Signos y síntomas
 Zona afectada enrojecida.

DIAGNOSTICO
se basa en el cuadro clínico. No es necesario el aislamiento
microbiológico en el foco de la infección, a menos que se sospeche
bacteriemia o coinfección con otros microorganismos. Las pruebas
bacteriológicas también deben realizarse si la lesión evoluciona hacia
un absceso o flemón, y en casos de infección intrahospitalaria
(erisipela por herida).
Puede ser necesario realizar una ecografía venosa de la extremidad
afectada.
DIAGNOSTICO
DIFERENCIAL
Enfermedades cutáneas eritematosas con manifestaciones generales
● 1) infecciosas: celulitis (el eritema no está bien delimitado, el
infiltrado inflamatorio es más profundo, no hay signos de propagación
linfática), flemón (se caracteriza por estado general más grave, dolor
más intenso en el lugar afectado, se observa
fluctuación/desplazamiento/transfusión del contenido purulento),
herpes zóster, erisipela, eritema migratorio en el curso de la infección
por Borrelia burgdorferi;

● 2) no infecciosas: trombosis venosa profunda, eritema nodoso,


dermatitis de contacto, carcinoma erisipeloide (inflamación en el
tumor neoplásico, con mayor frecuencia en el cáncer de mama)
PREVENCION
Mantenga la piel sana evitando la piel seca y previniendo cortaduras y
raspaduras. Esto puede reducir el riesgo de sufrir erisipela
TRATAMIENT
• Antibióticos orales o IV O
Los antibióticos de elección para la erisipela incluyen los siguientes :
• Tratamiento sistemático de primera línea: penicilina V 500 mg por vía oral 4 veces al día
durante ≥ 2 semanas
• Terapia oral alternativa (p. ej., para pacientes alérgicos a la penicilina): eritromicina 500 mg
4 veces al día durante 10 días (sin embargo, la resistencia de los estreptococos a los
macrólidos está aumentando)
• Tratamiento parenteral de primera línea (para casos graves): penicilina G 1,2 millones de
unidades IV cada 6 horas, seguida 36 a 48 horas después por terapia oral con penicilina V
500 mg 4 veces al día
• Terapia parenteral alternativa (p. ej., para pacientes alérgicos a penicilina): ceftriaxona 1 g
IV cada 24 horas o cefazolina 1 a 2 g IV cada 8 horas, seguida 36 a 48 horas después por
terapia oral con eritromicina 500 mg 4 veces al día durante 5 a 10 días
• Infecciones con S. aureus: sensible a la meticilina: dicloxacilina 500 mg por vía oral 4 veces
al día durante 10 días
COMPLICACION
ES
La erisipela puede ser recurrente y causar linfedema
crónico. Las complicaciones de la erisipela suelen incluir
tromboflebitis, abscesos y gangrena.
