INTAKE AND OUTPUT
Definition:
It is the MEASUREMENT and RECORDING of all fluid intake and
output during a certain period.

• PER ORAL • URINE
• IV INFUSION • LIQUID FECES
• FEEDINGS VIA NG TUBE • BLOOD PRODUCTS
• Anything liquid at room temperature • WOUND DRAINAGE OR ANY FLUID ASPIRATED

INTAKE AND OUTPUT
• I/O is gauged in milliliters (mL)
• To measure fluid intake, convert household measures such as glass,
cup or soupbowl into metric units.

I/O helps determine the patient’s fluid status
• Are they hydrated?
• Is there any fluid overload/deficit?
• Is there an obstruction?

Neutral Balance Input = output


Positive Balance Input > output
Negative Balance Input < output

Normal Urine Output (mL/day) in adults

Normal 800-2000
Decreased 500
Decreased 100
Increased > or equal to 2500-3000

IMPORTANT POINT

• Check for an Order!


• Review record and analyze trend in I/O measurement, VS, lab findings,
weight
• Inspect all IV fluids, drips, infusions
• Inform patient and family the plan for I/O measurement
• Keep a pen and paper at the bedside
• Report ant questionable output or drastic changes in the patients’ condition
Patient Be***in*os was admitted at Deseret NS2 dure to loose watery stool for more than
20 episodes per day and with a diagnosis of AGE with severe dehydration. I/O monitoring was
ordered qShift. Below is a list of the total consumption of the patient as observed by the Watcher
who is a Nurse.
