Over view of Nursing and medical diagnosis related to ischemic heart diseases
Ischemic heart disease (IHD) is a condition in which there is an inadequate supply of blood
It includes:
- Myocardial infarction
A 72 year old male patient admitted in hospital k/c diabetes HTN, IHD,CKD S/p CABG
DONE in2017 with chief complain of low Bp, abdominal distension, testicular swelling, pedal
edema, on physical assessment Gcs 15/15, BP 80/50, pluse 94, temp 98 F, Spo2 99%. During
the course of admission patient kept in icu invasive investigation was preformed CBC, UCE,
ABG’s and then started inotropic support, Pulmo taken on board ultra sounded tap done 1
litre fluid extracted. Patient responded well to i/v diuretics than started inj: mero and inj
leflox plus intropic support continued. Ultrasound kub done. Patient again shifted to the ward
after 3 days. Patient EF is 20% inotropic support continued and then patient discharged on
diuretics and midodrine tablet. Patient responded well to the treatment till now.
Nursing diagnosis :
Decreases cardiac output related to decrease blood supply as evidence by tachycardia and
fatigue.
Ineffective tissue perfusion related to narrowed arteries as evidence by decrease blood pressure.
