5. Reporting of FDS Implementation


• Observe regular reporting of FDS data using the attached templates with specific instructions and
timeliness. The report provides statistical and narrative information on the following:
a. Newly registered households provided with foundational FDS topics
b. Transitioning households provided with FDS on graduation.
c. FDS compliance and other sessions provided
d. Modules developed and/or localized
e. Inventory of Parent Leaders
f. Inventory of Gulayan sa Barangay
g. Efforts related to Transaction Accounts such as but not limited to Updates on submitted Re-Entry
Action Plan, HH’s compliance to Financial Literacy Sessions, coordination Meeting with
Landbank and / or other financial institutions, etc.
h. Efforts related to 0-5 children such as but not limited to Updates on submitted Re-Entry Action
Plan, HHs compliance to KNM sessions, coordination Meeting with Department of Health and / or
other health authorities, registration of HH in collaboration with the Beneficiary Data
Management, etc.
