Problems According to
Priorities
I. Characteristics of a Family Nursing Care Plan
• It focuses on actions which are designed to solve or
minimize existing problem
• It is the product of a deliberate systematic process
• It relates to the future because it is a document to
evaluate the quality you have rendered.
• It is based upon identified health and nursing problems
• It means to an end, not an end itself. There is not really an
end when taking care of a patient/family.
• It is a continuous process, nit a one-shot deal.
II. Desirable Qualities
1. Clear and explicit definition of of Nursing Care Plan
the problem

2. Realistic. You have to know not

just the ability of the family to
participate but the Nurse as well. 4. Useful in written form.

3. Prepare jointly with the family.

Active participation is greatly
appreciated.
III. Importance of Planning
● They individualize care to clientsCare

● Helps in setting priorities by providing information about the client as well as

the nature of his problem

● Promotes systematics communication among those involved in the health

care effort

● Continuity of care is facilitated through the use of nursing care plans

● Facilitate the coordination of care by making known to other members of the

health team what the nurse is doing

Goals and Evaluating
Problems Intervention
Objectives Care
Prioritize the Health Conditions and Problems

● Nature of the problem (health

threat, health deficit)
● Modifiability of the problem
(easy or hard to modify)
● Preventive (impact to the
family)
Define Goals and Objectives of Care
● Conditions to be observed to
show problem is prevented,
controlled, resolved or eliminated.
Client response or behavior
(expected outcome)
● Specific, Measurable, Client-
Centered
Statements/Competencies
Goal
● It should be realistic or attainable
● It is the best stated in terms of
client outcomes, whether the
individual, family or community
levels
● Tells where the family is going
after the interventions
Objective
● Specify the criteria by which the
degree of effectiveness of care are
to be measured
● Milestone to reach the destination
or the goal
● The more specific the objectives,
the easier is the evaluation of their
attainment
After nursing intervention, the family will be able:
● Feed the mentally retarded child
according to prescribed quantity
and quality of food
● Teach the mentally retarded child
simple skills related to the
activities of daily living
● Apply measures taught to prevent
infection in the mentally retarted
member
Short term or Immediate Objectives

● Immediate attention
● Short period of time
● Few family contacts
● Less resources
Short term or Immediate Objectives

Example: The sick member will take

the drugs accurately as to dose,
frequency, duration and drug
combination
● Objectives vary according to the
time span required for their
realization
Medium term or Intermediate Objectives
● Not immediately achieved
● Required long-term ones
Example: All member will have
medical check-up and laboratory
confirmation to diagnose malaria
● Those which are not immediately
achieved and are required to attain
the long-term ones
Long term or Ultimate Objectives
● Several nurse-family encounters
● Investment of more resources
Example: All members will carry out
mosquito vector control measures
● Require several nurse-family
encounters and an investment of
more resources
Develop Intervention Plan
Decide on Measures to help family
eliminate:
● Barriers to performance of health
tasks
● Underlying cause/s of non-
performance of health tasks
● Family-centered alternatives to
recognize detect monitor control
Develop the Evaluation Plan
• Specify: criteria/outcomes based on Objective of care
• Methods tools
