LIVER ABCESS

DEFINATION

A liver abscess is defined as a pus-filled mass in the liver that can


result as a complication of various intra-abdominal infections by
hematogenous spread via portal vein from the gastrointestinal tract
or may develop after traumatic injury to the liver.
TYPES

There are three major form of liver abscess, classified by etiology-


 PAYOGENIC LIVER ABSCESS-it is most often
Polymicrobial ,accounts for 80% of hepatic abscess cases.
 AMOEBIC LIVER ASSCESS- due to Entamoeba
histolytica ,accounts for 10% of cases.
 FUNGAL ABSCESS- most often due to candida ,accounts for less
than 10% of cases.
CAUSES

 Biliary tract disease


 Peritonitis
 Cholangitis
 Pyogenic abscess (Streptococcus ,Staphylococcus , Klebsiella,
Pseudomonas)
 Amebic infection caused by Entamoeba histolytica.
RISK FACTOR
 Poor sanitation and contaminated drinking water
 Age older than 70 years
 Medical conditions such as cancer , diabetes or week immune system
 Medicine ,such as steroids or chemotherapy
 Alcohol
 Poor nutrition
 Trauma due to blunt or penetrating injuries
PATHOPHYSIOLOGY
Due to etiological factors

Increase the permeability of the membrane in intestine

Bacteria and amoeba enter the capillary system of the liver

Local infection

Bacterial toxins destroy liver tissue

The result is an abscess cavity filled with pus , dead leucocytes ,


liquefied liver cell and bacteria
Liver Abscess
CLINICAL MANIFESTATION

 Fever Chills
 Nausea and vomiting
 Anorexia weight loss and weakness
 Right upper quadrant abdominal pain and tenderness
 Hepatomegaly
 Right pleural chest pain
 Jaundice
DIAGNOSTIC EVALUVATION

 Blood test: LFT,CBC,PT and blood culture


 Stool culture
 Chest and abdominal X-ray
 Radio nucleotide ( liver scan)
 Ultrasound
 CT Scan
TREATMENT

 Antibiotic
 Percutaneous drainage
 Surgical drainage

NURSING DIAGNOSIS

 Hyperthermia related to liver infection.


 Imbalance nutrition less than body requirement related to anorexia,
nausea vomiting as evidence by reduce body weight.
 Activity intolerance related to fatigue and lethargy.
