 A 37 Years old male patient came to OPD with complain of red
CASE: sores on the face, itching on the affected area and swelling in
both hands since 2 weeks.
SUBJECTIVE:
● NAME: SNP
● AGE: 37 Years
● GENDER: Male
● C/C: Red sores on the face, itching on affected areas, swelling on both the
hands since 2 weeks.
● PAST HISTORY: No past history is present
● FAMILY HISTORY: Not significant
● Patient have visited skin OPD on 8/12/23.

ASSESSMENT:

On the basis of signs of symptoms the

patient was diagnosed with “IMPETIGO”
GOALS OF THE THERAPY:

To reduce the swelling and itching

To prevent its spread to other people

and patient’s other body part

 ADVICE TO THE PATIENT:
• Wash hands regularly, keep skin clean and take regular baths.
• Avoid close contact with other members of family or in public.
• Do not share personal items .
• Wash the linens in hot water.
• Complete the treatment given even if the sores are healed.
• Do not touch the sores.
• Place a nonstick bandage over the area to prevent sores from
spreading.
