Pharmacologic/Therapeutic Category NSAIDs Agent Use/ Indications Ankylosing spondylitis, Osteoarthritis, Rheumatoid arthritis, Analgesia, primary dysmenorrhea, Acute pain due to minor strains, sprains, and contusions, acute gout; postoperative pain Pharmacological Effects Diclofenac is a phenylacetic acid derivative that is relatively nonselective as a COX inhibitor. It inhibits prostaglandin synthesis, to produce anti-inflammatory, analgesic, and antipyretic effects Adverse Reactions GI: abdominal pain or cramps, bleeding, constipation, diarrhea, flatulence, indigestion, melena, GU: nephrotic syndrome, acute renal failure, fluid retention, interstitial nephritis, oliguria, proteinuria. Hepatic: jaundice, hepatitis, hepatotoxicity. Metabolic: hypoglycemia, hyperglycemia. Respiratory: asthma. Skin: Stevens-Johnson syndrome, allergic purpura, alopecia, dermatitis, eczema, pruritus, rash, urticarial Precautions/Warnings/Contraindication avoid in acute porphyria Contraindicated for the treatment of perioperative pain after CABG surgery. Patients hypersensitive to drug History of asthma, urticarial or other allergic reactions on taking aspirin or other NSAIDs. Pregnancy category: B, D in third trimester Breast-feeding: contraindicated Interactions: Anticoagulants, including warfarin: May cause bleeding. Cyclosporine, digoxin, lithium, methotrexate: May reduce renal clearance of these drugs and increase risk of toxicity. Diuretics: May decrease effectiveness of diuretics. Avoid using together. Insulin, oral antidiabetics: May alter requirements for antidiabetics.
