OUR MISSION
Childrens Community Mental Health Services helps families struggling to care for children with intense emotional needs in their community, outside of hospital and/or residential placement. We strive to empower all families to access services that promote health and recovery in a way that is seen as strength based, family driven, culturally and linguistically competent and community based. We strive for families to receive services that improve quality of life for the children and those whose lives have been impacted by their mental illness. Childrens Community Mental Health Services (CCMHS) is part of the Child and Family Institute at St. Lukes Hospital and Roosevelt Hospital. CCMHS provides intensive mental health services to children throughout New York City to ensure that they are able to function in the community, outside of residential or hospital placement. WHO WE SERVE
CCMHS serves children who suffer from serious emotional disturbance or have a behavioral disorder. Our programs are parent-focused and provide individualized care to patients. We work together to ensure that your child is maintained safely in the community. We serve children between the ages of five and 17.9 who: Have been diagnosed with a Serious Emotional Disturbance (SED) Have a behavior disorder that interferes with his/her functioning in the community Are at risk for being hospitalized, re-hospitalized, or requiring residential placement Our services include: Blended Case Management for Children (BCM) The Childrens Single Point of Access (CSPOA) The Home and Community Based Services Waiver (HCBSW)

BLENDED CASE MANAGEMENT (BCM)


Blended Case Management (BCM) is a program designed to work with children who already receive clinical care (e.g., therapy, medication, etc.) but need something else in order to be successful at home, at school and in the community. BCM has two levels of services: Supportive Case Management (SCM) is the least restrictive level of care. The case manager will come see the child a minimum of two times per month. Intensive Case Management (ICM) is for children who require more services. The Case Manager will visit a minimum of three times per month plus one additional visit with a parent, teacher or other caregiver. Case Managers may work with families on a variety of issues including: Coordinating needed services within the community Supporting children and working with their families Educating families about various services and resources available in mental health care Helping families with streamlining their many appointments We provide tailored services to meet the needs of your child. Its uniqueness allows your child to fluctuate between intensive and supportive levels of service (as needed) without severing ties with familiar case managers.

HOME AND COMMUNITY-BASED SERVICES WAIVER (HCBSW)


The Home and Community-Based Service Waiver (HCBSW) program is the highest level of outpatient psychiatric care available to children outside of hospital and/or residential placement. Children are seen up to six times per month by an individualized care coordinator (ICC) and receive a variety of family-driven services including: Individualized care coordinator services 24-hour crisis response Intensive in-home services Respite care and skill building Family support Based on the array of services available for your child, you can see that the Waiver Program is very intensive. The services are designed to help your child in every facet of their lives. ICCs help families by: Utilizing available resources in the community Supporting parents with challenging issues Aiding parents with cross-system collaboration (school, mental health treatment, and after-school community programs) The Waiver Program is not for everyone. It requires a firm time commitment.

CHILDRENS SINGLE POINT OF ACCESS (CSPOA)


Childrens Single Point of Access (CSPOA) links families with services that are provided by St. Lukes Hospital and other community-based mental health programs throughout New York City. We serve children between the ages of five and 17.9 who suffer from serious emotional disturbance or have behavioral disorders that interfere with functioning in the community. The child may also be at risk for being hospitalized, re-hospitalized, or requiring residential placement. Our services are determined by the familys needs, borough of residency and program availability. Community-based mental health programs include: Home and Community-Based Services Waiver (HCBSW) Childrens Case Management Services (ICM, SCM and BCM) Family-Based Therapeutic Intervention (FBTI) Bronx Only Community Residence (CR)
