GUMBO APPLICATION ORGANIZATIONS OR GROUPS | SEPTEMBER 2012


Answer the following questions in a word doc & email it to: art.gumbo.dbq@gmail.com. Note that there is a maximum word count for each section. Please address all questions & adhere to word count. There is a small fee to submit application based on a sliding scale: $5-$20, you choose what you can afford. You will be notified via email if your application is eligible to compete for funding. In this notification you will be directed where to mail your application fee. Application Deadline: Saturday, September 15, 2012 by 11:59 p.m. Event: Thursday, September 20, 6-8 p.m., Voices Warehouse Gallery, 10th & Jackson Streets, Dubuque, IA. You must be present to win.

1. First and last names of the project leader and any collaborative partners 2. Contact information for project leader: email address, phone number, mailing address 3. Project title 4. Project summary describe the specific project you would like to accomplish with these funds. Include collaborators, medium (s), and what you intend to accomplish. (200 words max) 5. How will your project impact the Dubuque community? (100 words max) 6. If awarded, how will you present your accomplishments at the next ART GUMBO in 3 months? (60 words) 7. If awarded, how will you use funding (up to $750) towards the realization of your project? Please include details of how the grant will be used. (100 words)
Be prepared to offer a brief verbal pitch of your proposal at the event. Project leader will give the pitch. No visuals (or collaborative partners) allowed during pitch.
