***Please note: a Letter of Explanation is required for a score less than 620

Zero Loan Fraud Agreement IRS Form W-9 Balance Sheet current within the last 90 days Profit & Loss Statement - current within the last 90 days PRMG Broker Compensation Plan - fully executed with appropriate compensation selection marked
