The Kindergarten registration process has now begun!!

If you have a child that is five years old on or before October 1, 2013, he/she is eligible for kindergarten. Please contact the LDW Main Office now to put your childs name on the Kindergarten registration list. In February/March, you will be receiving information regarding the registration dates for Kindergarten orientation, screening, vision and hearing screening. Kindergarten orientation and screening will be scheduled for late spring. At this time, we are collecting registration paperwork for all children attending Kindergarten in the Fall of 2013. Registration forms can be found on the Lester D. Wilson School website: www.alexandriaschools.org. On the home page please click on Student/Parents tab. Then click on forms & Important Documents. The required forms are: 1. Student Registration card 2. Student Profile 3. Medical forms including Universal Physical form 4. Medical History form 5. NJ dept. of Health Recommended Vaccines (FYI please review) Send a copy of your childs current immunizations even if they are NOT YET COMPLETE. If your child is scheduled for a physical after the April 1st registration date, please give us what you have now and update the office with the new forms/information as soon after the appointment as possible. Proof of Immunization requires your health care practitioners signature. 6. Photo Release Form 7.
