Registration forms are available on payment of Rs. 300/- (for Balco employees) and Rs. 1200/- (for Non Balco employees) in the School office during 08.30 a.m. to 12.30 p.m. on the above mentioned dates. 2. Balco employee should submit the service certificate issued by the H.R. department mentioned with Employees No. and grade. 3. Registration against EWS quota (residents within 3 K.m. of school area only) are welcome. Please contact the school office between 1.00 pm to 2.00pm from 18.02.2013 to 23.02.2013. Submission of documents: i. Date of Birth certificate of the candidate issued by Registrar of Births & Deaths. ii. Original service certificate in case of Balco employee should be issued during the form registration period. For EWS quota: Any two proofs of residential address- Electoral Identify card, BPL Ration card, Latest paid Electricity bill, Registered lease deed made one year prior to the submission of registration form along with an Affidavit on stamp paper should be submitted in case of rental accommodation. Last income certificate of the earning parent(s). Issued by the Revenue Officer should not be below the rank of Tehsildar, Balco/BPL Ration card. Adequate proof like ration card or Electoral identity card in support of their stay in Balco for the last one year. If the information provided is found to be incorrect or false the candidate shall be automatically debarred from the selection process without any correspondence in this regard. N.B.: Final admission is subject to verification of the above documents in original.
