    * AFFIDAVIT OF COUPLE LIVING-IN FOR MORE THAN 5 YEARS (Article 34) - Municipio or City Hall * MARRIAGE CONTRACT (if civilly married) * DEATH CERTIFICATE of Deceased Spouse (for widow/widower) 6. 7. 8. 9. PUBLICATION OF BANNS - 3 Sundays PRE-CANA Seminar - to be scheduled Canonical Interview with Parish Priest - Canonical investigation form before the interview. Confession - 1 or 2 day/s before the wedding.

You can bring your own flowers and we can refer you to our church choir or you can invite your own choir. Notes: 1. All requirements must be submitted at least two (2) weeks before the wedding. 2.
