    AFFIDAVIT

    I, Sri / Smt. / Miss ______________________________, aged about _____ years, S/o., W/o., D/o. ______________________________________ resident of village / town _________________, P.O. _________________, Dist. ____________ (Odisha) do hereby solemnly affirm and declare as under: That, I have applied for the post of Shikshya Sahayak in Jharsuguda Revenue/Education District only. That, I have not made application for engagement as Shikshya Sahayak in any Education District / Revenue District other than Jharsuguda Education/Revenue District. That, if on verification, it is found that, I have applied in more than one Education District / Revenue District then my application shall be summarily rejected without assigning any reason. That, I swear this affidavit to produce the same before the competent authority for their record and reference. That the fact stated above are true and correct to the best of my knowledge and belief.
