He belongs to the UK Psoriasis Forum that has over 3000 members and every spare moment he gets he is sending and receiving email messages giving advice and taking advice from people that have only just got it or people who feel like giving up life because they just cannot cope with it!! But at last by a stroke of luck or shall we say an answer to a prayer he came across ACV. He has been taking it now for 6 weeks and only after 3 weeks "certain things were starting to happen"!! He has P, as its known to people with it,on his scalp,in his ears,on his neck,on his chest,on his arms,on his elbows,on both his thighs,on his knees,on his bottom and his privates!! But now the raised red scaly scabs are starting to disappear and the areas of red are thinning out and small amounts of normal skin are taking there place.When ever he shaves he always used to cut the scabs on his chin and then he would bleed. That bleeding stopped yesterday after he shaved!! Other things have happened to him quite recently. He has stopped snoring and when he wakes up in the morning he can get straight out of bed and feel refreshed.He phoned me up yesterday evening to say that "he just could not believe it" but his grey hair has started to turn back into black its original colour!!! He has posted a message on his P Forum stating all what I have stated here and he has put photos of his body parts showing what has happened but only a few members have taken any notice!! So I am hoping who ever reads this memo of mine and knows someone that has P that they inform them of it as soon as possible. The Cyder Vinegar that we both use is Aspall Organic Cyder Vinegar. I was devastated but paid no mind to it thinking it would go away on its own. Months started to pass and the itching and flaking were beyond control. I felt like the lowest form of a human being. I had skin peeling on my nose and small patches on my chest, sides, arms, legs, scalp, ears & a tiny one on the left cheek. I felt so miserable. I would pass by kids running around and people doing casual everyday things and I would feel so depressed that I couldnt do those things without having to worry that someone is looking at me wondering what the heck that is in my ear or on my head or on my arm. Thats when I finally said enough is enough, there has to be some way to get rid of this thing. I bought all the non prescription treatments out there for the scalp and patches. They did little to help. Although 1 scalp treatment product lessened the patches on my head, it went from huge patches to really tiny patches. As i have continued to use these products and seeing little to no results, I finally accepted my fate and realized I might have to stay this way for the rest of my life. Almost a year or so has passed and about 2 weeks ago I was surfing this forum when I saw several posts about ACV. Not knowing what it was, I proceeded to read the posts and finally I figured out it was (Apple Cider Vinegar). So I set out to my local grocery store and could only find Richards ACV instead of the suggested Braags. I figured it was better than nothing and started on the treatment of 2 teaspoons mixed with honey (in my case i used sugar since I didnt have honey). 2 weeks later here I am, VERY HAPPY and giddy :). The ACV treatment is working. The patches are diminishing. They are no longer rough and flaky, but rather smooth, REGULAR HEALTHY skin is now there (only thing that remains is a mark where the patch once was, this should be easy to get rid of with cocoa butter or some kind of skin cream). The smaller tiny patches have completely healed up like popping a pimple.. Just a mark remains and the ones on my arms are beginning to heal up very nicely. I truly believe that without the internet and these forums, I would still be suffering as I was almost a year ago. With the ACV Treatment and doing the following Ive managed to see these results in less than 2 weeks. I did the following: 1) When a psoriasis patch popped up, I didnt scratch it at all, no matter how tempting. 2) I went to the doctors and got a topical ointment to put on the psoriasis. 3) I ate only vegetables and fruits, drank lots of water, and exercised frequently. 4) Remembered that my mind also had a involvement in my psoriasis and tried to stay

There are different types of psoriasis. They are :


Plaque psoriasis - Patches of raised, reddish skin covered by silvery-white scale Psoriatic arthritis - Which causes inflammation of the joints. Guttate psoriasis - Small, red spots on the skin Pustular psoriasis - White pustules surrounded by red skin Inverse psoriasis - Smooth, red lesions form in skin folds Erythrodermic psoriasis - widespread redness, severe itching, and pain

Causes of Psoriasis
The exact cause of psoriasis is not known. It is believed that a combination of several factors contributes to the development of this disease. Some common causes of psoriasis are the following:

Psoriasis occurs when the immune system overreacts, causing inflammation and flaking of skin. Abnormality in the mechanism in which the skin grows and replaces itself causes psoriasis. Psoriasis develops when the immune system tells the body to over-react and accelerate the growth of skin cells.

Definition
Psoriasis affects one to two per cent of the population in the United Kingdom (i.e. upwards of a million people in the UK). It is a chronic relapsing condition, mild to moderate involvement can usually be managed in primary care. The degree of psychological and social disability that accompanies psoriasis is commonly under estimated by the medical profession and this can result in sub-optimum care. There is no cure for psoriasis although there are effective suppressive treatments aimed at inducing remission or making the amount of psoriasis tolerable for the patient. For the majority of patients, the disease follows a chronic course interspersed with periods of remission. Relapses are difficult to predict.

Clinical Features & Differential Diagnosis

Well defined, circumscribed, varying sized plaques of erythematous (described as "salmon pink") skin summonted by varying thickness of silvery scale; most often located on elbows and/or knees; any skin siteI can be affected (see clinicasl features inindividual subsections on psoriasis types below). In the scalp there may be very thick scales. The finger (and toe) nails may be deformed with thickening, pitting and discolouration. Differential diagnosis includes eczema (especialy discoid and seborrhoeic eczema); tinea (fungus) infection; rarely cutaneous Tcell lymnphoma.

Suggested Investigations
The physician should make the patient aware of the possible therapeutic options including the simplest available therapies. The need for treatment will often be dictated by the patient's own perception of his/her disability. In order to advise the patient on suitable therapies, it is important to know the sites of involvement. Certain drugs are thought to precipitate or worsen psoriasis including alcohol, beta blockers, non-steroidal anti-inflammatories, some ACE inhibitors. Oral administration of lithium, chloroquine or mepacrine can be associated with the severe deterioration of psoriasis. It is helpful to record patients' views of the most upsetting aspect of their psoriasis so that management strategies can be directed appropriately within the therapeutic limitations based on risk benefit ratio. There are several forms of psoriasis and an individual during his or her lifetime may move from one type to another and the extent of involvement can range from trivial to almost total coverage. Psoriasis can change from stable plaque to unstable patterns, typified by eruptive inflammatory lesions easily irritated by topical treatments. Rarely is the diagnosis difficult as all practitioners will be familiar with the usual patterns of psoriasis but the type of psoriasis does influence the treatment as set out below.

Clinical Management in Primary Care


Psoriasis types and their treatments: For all types of psoriasis it is worth using a moisturiser/emollient which as well as reducing, dryness, scaling and cracking, may have a measurable anti-proliferative effect in psoriasis e.g. Aqueous Cream, liquid paraffin/ white soft paraffin 50:50 mix, Unguentum M. Emollients in the bath can include tar, e.g. Polytar emollient, Baltar (tar has anti-inflammatory properties). Chronic stable plaque psoriasis This is the most common type appearing on elbows, knees, sometimes more extensively over trunk and limbs. First line treatment: Vitamin D analogues eg. Calcipotriol Dovonex;. These are odourless and colourless, so cosmetically acceptable. Not suitable for face, flexures, genitalia (Tacalcitol Curatoderm. may be used cautiously on the face and in flexures). May cause irritation initially. Need to be applied thickly. Maximum application per week 100 120 G. Preparations usually applied once or twice daily. Expect improvement to be gradual with maximum benefit over eight to twelve weeks. Can be used long term or intermittently. Partial benefit may be all that is achieved (e.g. flattening or partial clearance). These preparations are less effective if the psoriasis is very hyperkeratotic/thickened (see below) Second line treatment: Tar preparations, e.g. Exorex, Psoriderm (as topical preparations in addition to tar in the bath). Tar products have anti-inflammatory properties in psoriasis. In order to lessen the cosmetic unacceptability of smell and staining purified tar preparations are used. Unfortunately as increasing

purification takes place so effectiveness is reduced. Preparations listed tend to be purified. This is very much a last resort treatment. The improvement with tar is slow. The preparation can be applied twice daily but is best used at night, covered with old pyjamas etc. to prevent staining of bed clothes. Third line treatment: Dithranol (see short contact Dithranol patient advice sheet). An excellent preparation and is probably the most effective topical treatment for plaque psoriasis but its disadvantages are that it irritates, needs to be used accurately and carefully, stains both skin and clothing and is not suitable for flexural skin sites or the face. If compliance is good psoriasis will clear completely over four to eight weeks. Preparations include Dithrocream, micanol (purified dithranol less staining) and Psorin (tar/salicylic acid/dithranol proprietary mixture). Topical Streoids: Are not usually recommended for treatment of chronic plaque psoriasis as the skin can become thinned and the psoriasis may become unstable. However, a proprietary Vitamin D3 analogue/steroid mix is available (Dovobet) and can be used for 4 weeks for revatively limited psoriasis but should NOT BE USED FOR MAINTENANCE. Whilst the treatment response for Dovobet is quicker than for Vitamin D3 analogues alone, but the degree of clearance beyond 4 weeks is not significantly increased. Chronic plaque psoriasis but particularly thickened. Pre-treat skin with salicylic acid e.g. salicylic acid ointment, BP (2%) to reduce scaling which otherwise acts as a barrier to prevent penetration of active ingredients in other preparations. If BP formulation difficult to obtain, tar/salicycilic acid mixture (eg Psoriderm) will fulfill the same function. Usually twice daily for one week with be sufficient. Proceed as for standard plaque psoriasis. Inflamed Psoriasis If psoriasis angry, sore or itchy, use moderately potent topical steroid, e.g. Eumovate ointment each day for one week, then introduce Calcipotriol ointment (preferable to cream as more effective) or weak tar. Withdraw topical steroid. Scalp psoriasis. If mild use tar shampoo (e.g. Polytar, Capasal,T-gel) plus either Calcipotriol scalp solution once or twice daily if psoriasis uninflamed. If scalp inflamed or itchy use topical steroid lotion, e.g. Betnovate lotion, Betamousse, Diprosalic, Synalar gel once or twice daily. (Warn patient to make sure lotion/gel applied sparingly so it does not run down forehead and face). A Vitamin D3 analogue/topical steroid mixture scalp application (Xamiol) is now available - the scalp equivalent of Dovobet; this can be used over-night but when shampooing out in the morning, the hair should be shampooed DRY - just shampoo, no water then rinse out normally; use for upto one month then have a break of 1 month before reusing. Thick scalp psoriasis. It is best to remove scale over night by applying Cocois liberally over night (covered with a shower cap, both to protect bedding and to enhance effect by hydrating scalp and softening the scale). Wash out with tar shampoo in the morning as above and then use as for standard scalp psoriasis by day (Calcipotriol or steroid lotion or combination). If scale very adherent, apply Dermol 600 for 15-20minutes, massage into scalp to loosen scale then removed with fine toothed comb before washing with tar shampoo. Xamiol can be used by day (as for scalp psoriasis above as an alternative to Calcipotriol or steroid lotion) in conjunction with the overnight tar application but do not wash out before applying the tar. Facial psoriasis. Use emollients and if that is insufficient add moderate potency topical steroid ointment (e.g. Eumovate) over night or tar/steroid combination (e.g. Alphosyl HC) overnight. Guttate psoriasis (numerous scaly droplet lesions over trunk which may follow streptococcal infection most common in children and adolescents and self-limiting after four to six months). Widespread lesions best treated by emollients, Vitamin D3 analogue cream/ointment, +/- topical tar and tar solution in bath (preparations as above). Check throat swab and if strep present treat with systemic antibiotic. Flexural psoriasis (psoriasis in axillae, groins, natal cleft, sub-mammary etc. will look different because the scaling is reduced in hydrated areas). Most commonly seen in the elderly. May be secondarily infected with yeasts.

Treat with moderately potent topical steroid antibiotic combination (e.g. Trimovate cream overnight). Some patients will tolerate tar preparations or Vitamin D3 analogues but in the majority such preparations will cause soreness. Psoriasis of palms and soles (often misdiagnosed as eczema). Usually very well demarkated and usually symetrical. May present as pustular psoriasis (green/yellow pustules and red/brown macules amongst well demarcated scale). Exclude fungal infection (take mycological samples). Treat with potent topical steroid plus or minus salicylic acid, e.g. Diprosalic ointment by day. Addition of tar at night may help, e.g. Exorex (cover with gloves plus or minus socks to prevent staining of bedding. Treatment should be directed to controlling symptoms but warn patient that treatment usually does not clear rash completely (especially with pustular psoriasis). Psoriatic nail dystrophy Unfortunately no good treatments for this but worth trying Vitamin D.3 (eg Dovonex) ointment rubbed into nails over night. Children: Apart from tar and dithranol preparations, the only other preparation licensed for use in children (over 6 year Psoriatic arthropathy Not common. Best treated with standard NSAID's but these may worsen psoriasis.

Reasons for Specialist Care


Reasons for specialist referral Diagnostic uncertainty. Failure of appropriately used topical treatment for a reasonable period of time e.g. two to three months. Very extensive disease. Apparent requirement for potent topical steroid (topical steroids not usually indicated other than for very short periods or specific sites/patterns). Generalised erythrodermic or generalised pustular psoriasis (emergency referral is indicated). Extensive, severe or disabling psoriasis. Occupational disability or excessive time off work or school. Content of referral letter should include: Reason for referral What is hoped to be gained from the referral All previously used therapies and quantities should be specified including side effects Relevant medical background information (including all patient's current medication) Information regarding patient's domestic circumstances (relevant when requiring application of topical Patient's ability to attend for hospital treatment, etc.

Medicines for psoriasis are often invaluable in controlling symptoms. But there are plenty of natural ways to treat psoriasis, too. Try these eight tips to help manage your psoriasis at home.

Psoriasis Self-Care Tip 1: Keep Skin Moist


Using moisturizer is an important part of self-care for psoriasis. Dry skin makes irritation and itchiness caused by psoriasis worse than it needs to be. A vicious cycle can result: dry skin can cause itching and scratching, leading to new psoriasis patches developing on the irritated skin. The best way to keep skin moist? Apply thick ointments like petroleum jelly (Vaseline) or heavy skin creams everywhere you can reach. Pat yourself dry and put on ointment immediately after getting out of the shower. Also, try super-moisturizing areas of skin affected by psoriasis. At bedtime, cover a patch of skin with ointment, and wrap it with a bandage or plastic wrap overnight. In the morning, wash the area gently. Over time, this can reduce scaling.

Psoriasis Self-Care Tip 2: Bathe, but Beware


Bathing can be another important part of psoriasis self-care, yet frequent bathing can also dry skin out, causing problems. A few tips for bathing with psoriasis:

Avoid hot water, which can irritate skin. Use lukewarm water instead. Dont towel off -- pat yourself dry, then apply moisturizer. Pure water dries skin. Add bathing salts, oil, or oilated oatmeal to bathwater to make it more skin-friendly.

Psoriasis Self-Care Tip 3: Stick to the Plan


It can be messy and inconvenient to slather thick goo onto psoriasis patches, day after day. But consistency is key. If youve been prescribed topical therapy, making it part of your routine can improve results. Also, topical therapies avoid the potential side effects of pills or injection treatments for psoriasis.

Psoriasis Self-Care Tip 4: Get Some Sun, With Care


The ultraviolet light found in sunlight is a proven treatment for psoriasis, but sunburns can make psoriasis worse. Cover unaffected skin with SPF 30 sunscreen and start with some limited sun exposure. Twenty minutes a day for three days a week can be a good start. Talk to your doctor first, because certain psoriasis medicines arent safe with high sun exposure.

Psoriasis Self-Care Tip 5: Quit Smoking

You can add psoriasis to the long list of health conditions made worse by smoking. In one study, people who smoked more than a pack a day were twice as likely to have severe psoriasis as those who smoked half a pack or less. Smokings negative effect was even stronger in women with psoriasis. Experts agree that quitting smoking is one of the best things you can do to care for your psoriasis. Dont forget that quitting smoking also reduces your risk of heart disease, lung disease, cancer, and a host of other health conditions.
