VERIFICATION AND APPEAL FORM

For use by undergraduate and postgraduate taught students appealing an academic decision of an Examining Board

GUIDANCE NOTES Before completing this Verification and Appeal Form, you should read the University Verification and Appeals Procedure which is to be found in the Academic Regulations Handbook on the web (www.cardiff.ac.uk/7253). Please ensure you understand the requirements of each ground before submitting your appeal as the information cannot be amended later. Advice and Guidance You are strongly advised to seek independent advice on the submission of academic appeals before completing this form. Advice can be obtained from the Cardiff University Student Union Advice and Representation Centre (ARC) , Park Place, Cardiff or telephone 02920 781410. Format and Time Scale for Submission Appeals must be submitted in hard copy to the Student Case Services (see bottom of page for address). Appeals will not be accepted in any other format and have to arrive no later than FOURTEEN DAYS (14) from the date of the letter formally notifying you of your result . All evidence must be attached to the appeal form and where evidence is not available at the time of submission, students must indicate on the form when the evidence will be presented. You will not normally be permitted to submit additional evidence later in the process. Admissible and Inadmissible Grounds The Grounds of Appeal that are admissible under the Verification & Appeals Procedure are: Arithmetical or other error of fact; Defects or Irregularities in the conduct of the assessment or advice relating thereto; Extenuating circumstances. The University does not accept Appeals which question the academic judgement of examiners, or appeals on any grounds other than those stipulated in 15.3 of the Procedure. In addition to this the University does not permit students to request for assessments to be re-marked as the academic marking processes are robust and overseen by External Examiners. Appeals submitted on these grounds will be rejected. I wish to seek verification of the decision of the Examining Board on the following grounds that (tick one or more relevant boxes). SECTION 3 Extenuating Circumstances 3.1. Please outline the extenuating circumstances which you claim impacted upon your performance (in the modules identified in Section 1) and the evidence you have presented in support of your claim.

In August, during the two weeks before my exam, I had been suffering from irregular appendicitis pain, due to which I was unable to put in my best effort in

the preparation, and thus performed poorly in the exam. On the morning of the exam, just before it started, I had taken a strong dosage of painkillers and antibiotics . Due to the strong medication, I felt very drowsy, restless and uncomfortable during the entire exam. I blanked out during the exam and couldn't write my answers to the best of my ability. My medical reasons, along with my personal problems, made it almost impossible for me to concentrate. Earlier, I was very apprehensive in disclosing my family matters in the extenuating circumstances form, but there was so much tension and gloominess in my house that my mind was preoccupied with family matters most of the time. My parents are going through a very difficult phase and are considering legal separation. This has been an utter shock to me for the past 3 months and I havent been able to handle this news well. It worsened in August when my parents got separated, not legally, but started living separately. I thought every couple has their share of problems, but I guess I just wasnt aware of the seriousness and severity of their problems, and I havent been able to accept it, as I never ever imagined it would go so far. Critical financial problems and failure of my fathers business, led to further tension and complications in my family.

If YES, please outline who you reported the extenuating circumstances to, the information you disclosed and the independent evidence you presented?

The Board was aware of my medical problems, as I had filled the extenuating circumstances form before my resit result was out. It was backed by my medical certificate and prescriptions as evidence. However, I had chosen not to disclose my family matters in the form.

(Students must show good reason why such circumstances could not have been made known to the Examining Board when the student was assessed).

I was unable to report my personal problems to the Chair, before the declaration of my result, as I was emotionally imbalanced and very sad. I didnt want to disclose such personal problems to anyone and didnt feel comfortable at all in talking about my parents. I was very hesitant to talk

about it, until I felt it was absolutely necessary to share them with the board, and thus disclosed only my medical problems in the extenuating form.

4. Please indicate what you would like to happen as a result of your appeal: I plead the board to reconsider my circumstances and give me one last chance to get promoted to second year. I would like to sit for a supplementary re-sit or to be allowed to carry forward this module to next year and be allowed to appear for this module next year, along with the second year modules. If none of these two options are possible, I humbly request the board to trust my capabilities and grant me with pass credits. If I repeat the year externally, I will have to sit idle for 1 whole year as in India it is extremely difficult for an undergraduate to find productive work. If I am not able to get promoted to second year, my parents are of the opinion that I return home and think of an alternative career, but after the two years I have spent in your esteemed University, I do not want to drop out or quit now, as I know I can and will perform much better than this.
