Immersion Program Guidelines for Students in the Related Learning Experience 1. Provide your parents/guardian a copy of the guidelines of the program. 2. Secure a parent/guardian admission form going for immersion. 3. Be in your Community Health Nursing uniform with your nameplate and ID at all times 4. For health care activities, wear rubber shoes, black/blue maong pants with a shirt, will be allowed for certain activities (e.g., field work). 5. Proper decorum is expected as a bona fide student. Remember, you are representing yourself, your family, and the AdDU School of Nursing. 6. Two students will be allowed to stay with a foster family in their assigned community at a minimum of _____________________ for __________________ (two female students or two male students). 7. Bring your own provisions when you stay with your foster family (e.g., food, medicine, clothes, etc.). 8. Intake of alcohol / smoking / gambling in any form is strictly prohibited in the community. 9. What to bring: a. CHN uniform b. Sleeping clothes (cotton pajamas only, no sexy lingerie or nighties). c. 1 light linen d. 1 robe e. 1 bath towel f. Underwear j. RLE-CHN paraphernalia (one for each pair) k. Wristwatch l. Slippers

m. Extra shirt/s n. Sleeping bags(optional) o. Water p. Food (rice, canned goods) q. Small pillow r. Insect repellant / mosquito net

g. Toilet articles h. Flashlight (extra batteries) i. Cellphone

In case the foster family does not have kitchenware/s, students should bring their own: a. Spoon and fork b. Can opener c. Plate/s d. Drinking glass
